What it is used for

Treatment in prostate cancer with bone metastases is indicated when the cancer has progressed after at least one hormonal therapy.

How it works

Zoledronic acid belongs to the bisphosphonate group of medicines. It binds strongly to mineralized bone and inhibits osteoclasts, the cells responsible for breaking down bone tissue. Cancer can accelerate this bone-resorption process, releasing calcium and weakening affected bones. By reducing osteoclast activity and bone resorption, zoledronic acid helps lower cancer-related high calcium levels and reduces skeletal complications associated with malignant bone disease.

How it is taken

Zunitra is intended for intravenous use by a healthcare professional and should not be self-injected. Comparable zoledronic acid 4 mg/5 mL concentrates require dilution before intravenous infusion and must not be given as a direct injection. Zoledronic acid doses should not exceed 4 mg per administration and infusion should take no less than 15 minutes because faster administration increases renal toxicity risk. Kidney function should be assessed before treatment, and hydration and electrolyte status require clinical attention.

Important warnings

Zoledronic acid can cause serious kidney injury, including renal failure; renal function should be assessed before treatment and dehydration and other kidney-toxic medicines require particular caution. Hypocalcemia must be corrected before treatment, and calcium, phosphate, magnesium and renal function may require monitoring. Osteonecrosis of the jaw has occurred, particularly in cancer patients receiving intravenous bisphosphonates; a dental examination and preventive dental care are advised before treatment, and invasive dental procedures should be avoided during therapy when possible. Severe bone, joint or muscle pain and atypical femur fractures can occur. Serious hypersensitivity reactions, including anaphylaxis, have been reported. Zoledronic acid should not be given concurrently with another medicine containing zoledronic acid or another bisphosphonate. Aminoglycosides, calcitonin and loop diuretics may increase the risk of hypocalcemia, while nephrotoxic medicines may increase renal risk.

Side effects and monitoring

Common effects reported with oncology zoledronic acid include nausea, fatigue, anemia, bone pain, constipation, fever, vomiting and shortness of breath. Flu-like or acute-phase symptoms such as fever, headache, muscle or joint pain may occur, particularly around early infusions. Serious risks include kidney deterioration or renal failure, severe hypocalcemia, osteonecrosis of the jaw, severe musculoskeletal pain, serious allergic reactions and atypical femur fractures. Seek medical attention for facial or throat swelling, breathing difficulty, seizures or marked numbness or muscle spasms, persistent jaw pain or non-healing mouth sores, or new unexplained thigh or groin pain. Renal function and calcium and other relevant electrolytes should be monitored as clinically appropriate.

Pregnancy

Zoledronic acid can cause fetal harm and should not be given during pregnancy. Pregnancy status should be checked before treatment in patients who can become pregnant, and effective contraception is advised during and after treatment. Because bisphosphonates remain incorporated in bone and can be released over a prolonged period, potential fetal risk may persist after treatment has ended.

Breastfeeding

Breastfeeding is not advised during and after zoledronic acid treatment. It is not known whether zoledronic acid passes into human milk or affects milk production or a breastfed child, but serious adverse effects are considered possible and the medicine can remain stored in bone for a prolonged period.

Food and drink
