Medicine overview

Danogen 200 mg is an oral capsule containing danazol, a synthetic steroid with androgenic and hormone-suppressing activity. Danazol affects the pituitary-ovarian hormonal system and is used for conditions including endometriosis. Danazol is also used for fibrocystic breast disease when other measures have not been successful and for prevention of hereditary angioedema attacks. Because it can cause significant hormonal, liver, clotting, and pregnancy-related risks, treatment requires medical supervision.

What it is used for

Danazol is used to treat endometriosis that is suitable for hormonal treatment. It can also be used for fibrocystic breast disease when other measures have not provided adequate relief and to prevent attacks of hereditary angioedema, including swelling affecting the skin, abdomen, or airway. The appropriate indication and duration of treatment depend on the condition being treated and the prescriber's assessment.

How it works

Danazol suppresses the pituitary-ovarian axis, reducing the release of follicle-stimulating hormone (FSH) and luteinizing hormone (LH) and altering sex-hormone activity. In endometriosis, these effects make endometrial tissue less active and promote its regression. For hereditary angioedema, danazol increases levels of deficient C1 esterase inhibitor, helping reduce swelling attacks. It also has weak androgenic, or male-hormone-like, activity.

How it is taken

Danogen 200 mg is taken by mouth exactly as prescribed. Danazol treatment schedules differ substantially according to the condition, so the prescribed dose and frequency should be followed without self-adjustment. For endometriosis, treatment should begin during menstruation unless pregnancy has otherwise been excluded. If a dose is missed, take it when remembered unless the next dose is near; do not double the next dose. Food, particularly a high-fat meal, can substantially increase danazol absorption.

Important warnings

Danazol must not be used during pregnancy or breastfeeding. It is also contraindicated in unexplained abnormal genital bleeding, markedly impaired liver, kidney, or heart function, porphyria, androgen-dependent tumors, active or previous thrombosis or thromboembolic disease, and hypersensitivity to danazol. A pregnancy test is recommended before treatment, and non-hormonal contraception should be used because fetal exposure can cause androgenic abnormalities. Serious risks include blood clots and stroke, liver injury, peliosis hepatis, hepatic adenoma, and benign intracranial hypertension. Seek prompt medical attention for severe or persistent headache with nausea, vomiting or visual changes, symptoms suggesting a blood clot or stroke, or jaundice. Important interactions include warfarin, carbamazepine, cyclosporine, tacrolimus, antidiabetic medicines, and certain statins including simvastatin, atorvastatin, and lovastatin.

Side effects and monitoring

Commonly reported effects include acne, oily skin or hair, weight gain, flushing, sweating, vaginal dryness or irritation, menstrual changes or absence of periods, nervousness, and irritability. Androgenic effects can include increased facial or body hair, hair loss, and voice changes; some androgenic changes may persist after treatment ends. Serious or less common problems include liver injury, thromboembolic events, severe skin reactions, benign intracranial hypertension, and blood-cell abnormalities. Periodic liver-function testing is recommended, and hematologic monitoring may also be considered. Medical attention is needed for jaundice, severe skin reactions, symptoms of a blood clot or stroke, or headache accompanied by vomiting or visual disturbance.

Pregnancy

Danazol is contraindicated during pregnancy because exposure can cause androgenic effects and abnormalities in a female fetus. Pregnancy should be excluded before treatment, and a non-hormonal contraceptive method should be used during therapy. If pregnancy occurs during treatment, the medicine should be stopped and the prescriber contacted promptly.

Breastfeeding

Danazol is contraindicated during breastfeeding. Do not breastfeed while taking danazol.

Food and drink

Food can substantially alter danazol absorption. In pharmacokinetic studies, a meal containing more than 30 grams of fat increased danazol exposure and peak blood concentrations compared with fasting. Follow the prescriber's instructions about taking the medicine in relation to meals.

Will danazol stop my periods while I am being treated for endometriosis?

It can. Danazol commonly changes menstrual patterns and may cause amenorrhea, meaning periods stop. Ovulation and cyclic bleeding usually return after treatment ends, although persistent absence of periods has occasionally been reported.

Can Danogen cause weight gain, acne or oily skin?

Yes. Weight gain, acne and oily skin are recognized androgen-like effects of danazol. Tell your prescriber if these or other effects become difficult to tolerate.

Take the missed dose when you remember unless it is nearly time for the next dose. If the next dose is close, skip the missed dose and continue the prescribed schedule; do not take two doses together to compensate.

Does Danogen need blood tests during treatment?

Monitoring may be required. Periodic liver-function tests are recommended because danazol can affect the liver, and the prescriber may also consider monitoring blood-cell counts and other laboratory measurements.

Can I take Danogen with a statin or blood thinner?

Check with the prescriber or pharmacist first. Danazol can prolong the effect of warfarin and can increase the risk of muscle injury with certain statins, including simvastatin, atorvastatin and lovastatin.
