Medicine overview
Evalon Cream is an estrogen medicine containing estriol 1 mg per gram of cream. It is used intravaginally for estrogen-deficiency symptoms associated with menopause, especially changes affecting the vagina and lower urinary and genital tissues. Estriol replaces some of the estrogen that declines after menopause. The 15 g product is supplied in an aluminium tube with a calibrated applicator, which allows the prescribed amount of cream to be placed inside the vagina.
What it is used for
Evalon Cream is indicated for estrogen-deficiency symptoms due to menopause. It is particularly used when reduced estrogen causes atrophy of the lower urogenital tract. These changes may contribute to vaginal dryness, itching, discomfort during intercourse and other vaginal or urinary symptoms. Treatment is intended for postmenopausal women, including women whose menopause occurred naturally or following surgery.
How it works
Estriol is a naturally occurring estrogen and is relatively short acting. After menopause, lower estrogen levels can cause the vaginal and lower urinary tissues to become thin and less healthy. Intravaginal estriol replaces estrogen locally and helps restore the urogenital epithelium, normal vaginal microflora and physiological vaginal pH. These changes can reduce symptoms associated with urogenital atrophy, including vaginal dryness, itching and discomfort during intercourse.
How it is taken
Evalon Cream is administered inside the vagina using the supplied calibrated applicator, usually before going to bed. The applicator is filled to the marked ring as directed; a full marked application contains 0.5 g of cream, equivalent to 0.5 mg estriol. The frequency and duration should follow the prescriber's instructions. After use, the applicator should be washed with warm soapy water, rinsed thoroughly and not placed in hot or boiling water.
Important warnings
Do not use Evalon Cream with known, previous or suspected breast cancer; known or suspected estrogen-dependent cancer such as endometrial cancer; unexplained genital bleeding; untreated endometrial hyperplasia; previous or current deep-vein thrombosis or pulmonary embolism; known thrombophilic disorders; active or recent arterial thromboembolic disease such as angina or myocardial infarction; acute liver disease or unresolved abnormal liver function; porphyria; or allergy to estriol or another ingredient. Medical review is required for conditions including fibroids, endometriosis, hypertension, diabetes, gallstones, migraine, epilepsy, asthma, systemic lupus erythematosus and risk factors for blood clots or estrogen-dependent cancers. Seek prompt medical attention for painful leg swelling, sudden chest pain or shortness of breath. Treatment should be reviewed if jaundice, worsening liver function, a significant rise in blood pressure, a new migraine-type headache or pregnancy occurs. Medicines that induce estrogen metabolism, including some anticonvulsants, rifampicin, rifabutin and St John's wort, may reduce estrogen effects or alter bleeding patterns.
Side effects and monitoring
Reported effects include application-site irritation or itching, vaginal/cervical discharge, postmenopausal spotting, breast discomfort or pain, nausea, fluid retention and flu-like symptoms. These effects may be temporary, but persistent or troublesome symptoms should be discussed with a healthcare professional. Estrogen therapy is also associated with less common but potentially serious concerns including venous thromboembolism, ischemic stroke and estrogen-dependent cancers; the relevance of some systemic HRT risk estimates to low-potency vaginal estriol is uncertain. Unexpected or persistent vaginal bleeding requires medical assessment. Periodic clinical review should consider whether continued treatment remains appropriate.
Pregnancy
Evalon Cream is not indicated during pregnancy. If pregnancy occurs while it is being used, treatment should be stopped and medical advice obtained.
Breastfeeding
Evalon Cream is not indicated during breastfeeding. Estriol is excreted into breast milk and may reduce milk production.
