Medicine overview
Evomate 60 Mg Tablet contains raloxifene, a selective estrogen receptor modulator (SERM). It is an oral medicine used mainly to prevent and treat osteoporosis after menopause. Raloxifene acts like estrogen in some tissues, particularly bone, while blocking estrogen activity in other tissues such as the breast. This helps maintain bone density and can also reduce the risk of invasive breast cancer in certain postmenopausal women. Raloxifene is not recommended for premenopausal women.
What it is used for
Evomate is used for the prevention and treatment of osteoporosis in postmenopausal women. Raloxifene reduces the risk of vertebral fractures associated with postmenopausal osteoporosis, although a reduction in hip fractures has not been demonstrated. Raloxifene may also be prescribed to reduce the risk of invasive breast cancer in postmenopausal women with osteoporosis or those considered at high risk for invasive breast cancer.
How it works
Raloxifene is a selective estrogen receptor modulator. This means it can produce estrogen-like effects in some tissues while blocking estrogen effects in others. In bone, its estrogen-like activity reduces bone breakdown and helps preserve bone density after menopause. In breast tissue, raloxifene blocks estrogen activity, which helps reduce the risk of developing certain estrogen-sensitive invasive breast cancers. Its actions differ between tissues, so it is not the same as taking estrogen replacement therapy.
How it is taken
Evomate is taken by mouth as directed by the prescriber. Raloxifene 60 mg tablets can be taken at any time of day, with or without food. Taking the medicine at approximately the same time each day can help with consistent use. Do not take more or less than prescribed or stop treatment without discussing it with the prescriber. Adequate calcium and vitamin D intake may also be advised when dietary intake is insufficient.
Important warnings
Do not use raloxifene if there is an active or previous history of venous thromboembolism, including deep-vein thrombosis, pulmonary embolism, or retinal-vein thrombosis. Raloxifene increases the risk of blood clots and should generally be stopped at least 72 hours before and during prolonged immobilization. Seek urgent medical attention for unexplained leg swelling or pain, sudden chest pain, shortness of breath, coughing up blood, or sudden vision changes. An increased risk of death from stroke has been observed in postmenopausal women with coronary heart disease or increased coronary risk; individual cardiovascular and stroke risks therefore require consideration. Raloxifene should not be used to prevent cardiovascular disease and is not recommended in premenopausal women. Use requires caution with liver or kidney impairment and a history of estrogen-associated hypertriglyceridemia. Concomitant systemic estrogen therapy is not recommended. Cholestyramine and other anion-exchange resins can substantially reduce raloxifene absorption, while patients receiving warfarin or related coumarin anticoagulants may require monitoring when raloxifene is started or stopped.
Side effects and monitoring
Common effects reported with raloxifene include hot flashes, leg cramps, peripheral swelling, joint pain, sweating, and flu-like symptoms. Nausea and other gastrointestinal symptoms can also occur. The most important serious adverse effect is venous thromboembolism, including deep-vein thrombosis and pulmonary embolism. Obtain urgent medical care for new leg swelling or pain, sudden chest pain, shortness of breath, coughing up blood, or sudden changes in vision. Women should continue appropriate breast examinations and mammography, and unexplained vaginal bleeding should be medically evaluated. Patients taking warfarin or another coumarin anticoagulant may require prothrombin time/INR monitoring when raloxifene therapy changes.
Pregnancy
Raloxifene must not be used during pregnancy or by women who may become pregnant. It may cause fetal harm and is intended for use in postmenopausal women.
Breastfeeding
Raloxifene should not be used while breastfeeding. It is not known whether raloxifene is excreted in human milk.
Food, drink and interactions
No specific direct interaction between raloxifene and alcohol is established in the authoritative prescribing information reviewed. For osteoporosis management, avoiding excessive alcohol intake is generally advised.
