Medicine overview
Vagifem 25 mcg is a local estrogen treatment containing estradiol, an estrogen chemically and biologically identical to human 17β-estradiol. Each film-coated vaginal tablet contains estradiol hemihydrate equivalent to 25 mcg of estradiol. It is used after menopause to treat vaginal atrophy caused by reduced estrogen levels. The tablet is placed inside the vagina using its single-use applicator, allowing estradiol to act directly on vaginal tissues.
What it is used for
Vagifem 25 mcg is indicated for vaginal atrophy caused by estrogen deficiency in postmenopausal women. Vaginal atrophy can cause symptoms such as vaginal dryness, irritation and discomfort, including discomfort during sexual activity. Local estradiol replaces some of the estrogen that is lost after menopause and acts mainly in vaginal tissue. Experience with this 25 mcg product in women older than 65 years is limited.
How it works
Estradiol binds to estrogen receptors and restores estrogen activity in vaginal tissues. Estrogen promotes maturation of the vaginal lining, increasing superficial and intermediate epithelial cells while reducing basal cells. It also helps maintain vaginal pH near its normal range, supporting normal vaginal flora. These effects help reverse changes associated with postmenopausal estrogen deficiency and relieve symptoms of vaginal atrophy. Some estradiol is systemically absorbed after vaginal administration.
How it is taken
Vagifem 25 mcg is administered inside the vagina using the supplied applicator. The verified product information specifies an initial regimen of one vaginal tablet daily for 2 weeks, followed by one tablet twice weekly as maintenance treatment. Open the blister at the plunger end, gently insert the applicator into the vagina until resistance is met, press the plunger to release the tablet, then withdraw the applicator. If a dose is forgotten, take it when remembered without using a double dose. Follow the prescribed treatment instructions.
Important warnings
Do not use Vagifem with known, past or suspected breast cancer; known or suspected estrogen-dependent cancer; unexplained genital bleeding; untreated endometrial hyperplasia; previous or current venous thromboembolism such as deep-vein thrombosis or pulmonary embolism; known thrombophilic disorders; active or recent arterial thromboembolic disease such as angina or myocardial infarction; acute liver disease or unresolved abnormal liver function; porphyria; or hypersensitivity to its ingredients. Estrogen therapy requires consideration of risks involving the endometrium, breast, blood clots, stroke and other estrogen-related conditions. Stop treatment and seek medical assessment for jaundice or worsening liver function, a significant rise in blood pressure, a new migraine-type headache, pregnancy, or symptoms suggesting a blood clot or other serious reaction. The applicator can cause local vaginal trauma, particularly when vaginal atrophy is severe. Clinically important interactions are considered unlikely because systemic estradiol exposure is low, but enzyme-inducing medicines and St John’s wort may alter estrogen metabolism.
Side effects and monitoring
Commonly reported effects include vaginal discharge or discomfort, genital candidiasis/vaginitis, headache, abdominal discomfort, nausea, breast pain or tenderness, peripheral swelling and postmenopausal bleeding; some estrogen-related effects are more likely near the beginning of treatment. Serious estrogen-associated problems can include venous or arterial blood clots, significant unexplained vaginal bleeding, severe allergic reactions and estrogen-dependent cancers. Obtain prompt medical assessment for new or unexplained vaginal bleeding, symptoms of a blood clot, jaundice, marked blood-pressure elevation or a new migraine-type headache. Treatment should be reviewed periodically to determine whether it remains necessary.
Pregnancy
Vagifem is not indicated during pregnancy. If pregnancy occurs during treatment, Vagifem should be stopped.
Breastfeeding
Vagifem is not indicated during breastfeeding.
Food and drink
No product-specific food restriction was identified in the verified prescribing information. Vagifem is administered vaginally rather than swallowed.
Storage
Store below 25°C. Do not refrigerate.
Frequently asked questions
Can Vagifem help with vaginal dryness after menopause?
Yes. Vagifem 25 mcg is indicated for vaginal atrophy caused by estrogen deficiency after menopause, which can include vaginal dryness and irritation.
Does Vagifem work only locally, or does estrogen get into the bloodstream?
Vagifem is designed as local vaginal estrogen therapy, but some estradiol is absorbed into the bloodstream. Systemic exposure is lower than with conventional systemic estrogen therapy, but estrogen-related precautions still need to be considered.
What should I do if I forget a Vagifem tablet?
Use the forgotten dose when you remember, but do not use a double dose to make up for it.
Can Vagifem cause vaginal bleeding or discharge?
Yes. Vaginal discharge and discomfort are among the commonly reported reactions, and postmenopausal bleeding can occur. New, persistent or unexplained bleeding should be medically assessed.
Can Vagifem be used if I have had breast cancer?
Vagifem 25 mcg is contraindicated in women with known, past or suspected breast cancer according to the verified product information. A clinician should assess appropriate options for someone with a history of breast cancer.
Can I use Vagifem during pregnancy or breastfeeding?
No. Vagifem is not indicated during pregnancy or breastfeeding. Treatment should be withdrawn if pregnancy occurs.
How is the Vagifem 25 mcg tablet inserted?
Each tablet is supplied in a single-use applicator. The applicator is inserted gently into the vagina until resistance is met, the plunger is pressed to release the tablet, and the applicator is then withdrawn.
