Medicine overview

Femoral 60 Mg Tablet contains raloxifene, a selective estrogen receptor modulator (SERM). It is an oral medicine used mainly to prevent and treat osteoporosis in postmenopausal women. Raloxifene acts like estrogen in bone, helping reduce the bone loss that occurs after menopause. Raloxifene also has an established role in reducing the risk of invasive breast cancer in certain postmenopausal women, although it does not treat existing breast cancer.

What it is used for

Femoral 60 Mg Tablet is used for the prevention and treatment of osteoporosis after menopause. Raloxifene helps maintain bone density and reduces the risk of vertebral fractures. Raloxifene is also indicated for reducing the risk of invasive breast cancer in postmenopausal women with osteoporosis and in postmenopausal women considered at high risk of invasive breast cancer. It is not a treatment for existing invasive breast cancer and is not indicated to prevent breast cancer recurrence or noninvasive breast cancer.

How it works

Raloxifene is a selective estrogen receptor modulator. This means it can produce estrogen-like effects in some tissues while blocking estrogen effects in others. In bone, its estrogen-like action decreases bone breakdown and helps preserve bone mineral density after menopause. In breast tissue, raloxifene acts mainly as an estrogen antagonist, which helps explain its ability to reduce the risk of certain estrogen-sensitive invasive breast cancers in appropriately selected postmenopausal women.

How it is taken

Raloxifene 60 mg is taken by mouth once daily and may be taken at any time of day, with or without food. Taking it at about the same time each day can make regular use easier. If dietary calcium or vitamin D intake is inadequate during osteoporosis treatment or prevention, supplementation may be advised. Follow the prescribed instructions and do not take extra tablets to make up for a missed dose.

Important warnings

Do not use raloxifene if there is a current or previous history of venous thromboembolism, including deep-vein thrombosis, pulmonary embolism, or retinal vein thrombosis. Raloxifene increases the risk of blood clots and should normally be stopped at least 72 hours before and during prolonged immobilization, such as recovery after surgery or extended bed rest. Seek urgent medical attention for unexplained leg swelling or pain, sudden chest pain, shortness of breath, coughing up blood, or sudden vision changes. An increased risk of death from stroke was observed in a trial involving postmenopausal women with coronary heart disease or increased coronary risk, so individual stroke risk requires medical assessment. Raloxifene is not intended to prevent cardiovascular disease and is not recommended for premenopausal women. Use requires caution with liver or moderate-to-severe kidney impairment. Concomitant systemic estrogen therapy is not recommended. Cholestyramine substantially reduces raloxifene absorption, while patients using warfarin or other coumarin anticoagulants may require prothrombin time/INR monitoring when raloxifene is started or stopped.

Side effects and monitoring

Common effects include hot flashes and leg cramps; headache, peripheral swelling, nausea, joint pain, muscle pain, and other gastrointestinal symptoms have also been reported in clinical trials. The most important serious risk is venous thromboembolism, including deep-vein thrombosis and pulmonary embolism; retinal vein occlusion has also been reported. Obtain urgent medical help for leg swelling or warmth, sudden chest pain, unexplained shortness of breath, coughing blood, or sudden visual changes. Women using raloxifene for breast-cancer risk reduction should continue appropriate breast examinations and mammography. Prothrombin time/INR should be monitored when clinically appropriate in patients taking warfarin or related anticoagulants, and triglycerides may require monitoring in women who previously developed hypertriglyceridemia during estrogen treatment.

Pregnancy

Raloxifene is contraindicated during pregnancy and may cause fetal harm. It is not indicated for women of reproductive potential.

Breastfeeding

Raloxifene is not indicated for women of reproductive potential. It is not known whether raloxifene is present in human milk or what effects it may have on a breastfed child or milk production; its mechanism may interfere with estrogen functions in breast tissue during lactation.

Food, drink and interactions

No specific direct raloxifene-alcohol interaction is established in the prescribing information. For osteoporosis care, excessive alcohol consumption should be reduced because it is a modifiable factor affecting bone health.

Food and drink

Raloxifene can be taken with or without food, and no special meal timing is required. Calcium and vitamin D may be taken at the same time when supplementation has been advised.

Will raloxifene make hot flashes worse?

It can. Hot flashes are among the common adverse effects of raloxifene and are particularly likely to occur during the first months of treatment.

Are leg cramps normal with raloxifene?

Leg cramps are a recognized common side effect. New leg pain accompanied by swelling, warmth, or redness should not simply be assumed to be a cramp because raloxifene also increases the risk of serious blood clots.
