Medicine overview
Premarin 0.3 mg is an oral estrogen-only hormone replacement therapy containing conjugated estrogens. It is mainly used to replace estrogen when natural estrogen levels fall around or after menopause, helping manage symptoms such as hot flushes and menopausal vulvar and vaginal changes. It may also be prescribed for certain estrogen-deficiency states and for prevention or management of postmenopausal osteoporosis in appropriately selected women. The 0.3 mg product is supplied as a tablet for oral use.
What it is used for
Premarin 0.3 mg is used to treat moderate to severe menopausal vasomotor symptoms such as hot flushes and night sweats. It is also indicated for vulvar and vaginal atrophy caused by menopause and for hypoestrogenism associated with hypogonadism, castration, or primary ovarian failure. It may be used for prevention and management of postmenopausal osteoporosis in women at risk of fractures; when osteoporosis prevention is the sole reason for treatment, alternative non-estrogen medicines should also be considered.
How it works
Conjugated estrogens replace some of the estrogen that the body no longer produces in sufficient amounts. Estrogens act through estrogen receptors in tissues throughout the body and influence the reproductive system, bone, and other estrogen-responsive tissues. Restoring estrogen activity can reduce menopausal vasomotor symptoms and estrogen-related vulvar and vaginal changes. Estrogen also reduces the increased bone turnover that occurs after menopause, which helps limit loss of bone mineral.
How it is taken
Take Premarin by mouth exactly as prescribed. Swallow the tablet whole; do not crush or remove its coating. It may be taken without regard to meals. Treatment should use the lowest effective dose for the shortest duration consistent with treatment goals, with periodic review by the prescriber. Women who still have a uterus may also require a progestogen to reduce the risk of endometrial hyperplasia and cancer. Do not change or stop treatment without discussing it with the prescriber.
Important warnings
Do not use Premarin in patients with unexplained genital bleeding; known, suspected, or previous breast cancer except specific palliative indications; known or suspected estrogen-dependent cancer; active or previous deep-vein thrombosis or pulmonary embolism; active or previous arterial thromboembolic disease such as stroke or myocardial infarction; certain clotting disorders; significant liver disease where liver function has not returned to normal; hypersensitivity to the ingredients; or pregnancy. Estrogen therapy can increase risks including endometrial hyperplasia/cancer in women with a uterus, venous thromboembolism, stroke, and certain cancers. A progestogen should generally be considered when systemic estrogen is used in a postmenopausal woman with an intact uterus. Tell the prescriber about planned surgery and medicines or supplements because CYP3A4-modifying drugs, St John's wort, and several other medicines can alter estrogen exposure. Seek urgent medical attention for painful leg swelling, sudden chest pain, difficulty breathing, signs of stroke, severe allergic reaction, jaundice, or a new severe migraine-like headache. Unexpected or persistent postmenopausal vaginal bleeding requires medical assessment.
Side effects and monitoring
Commonly reported effects include breast pain or tenderness, breakthrough bleeding or spotting, vaginal inflammation, headache, nausea, abdominal symptoms, depression or mood changes, muscle or joint aches, leg cramps, and changes in weight. Serious or less common problems include venous blood clots, pulmonary embolism, stroke, heart attack, gallbladder disease, severe allergic reactions, liver problems, pancreatitis, and estrogen-sensitive cancers. Obtain urgent medical help for sudden chest pain, breathlessness, painful leg swelling, neurological symptoms suggesting stroke, or serious allergy. Regular clinical review and recommended breast screening are important, and unexpected vaginal bleeding should be assessed promptly.
Pregnancy
Premarin is not indicated for use during pregnancy. If pregnancy occurs during treatment, contact the prescriber.
Breastfeeding
Estrogens and their metabolites can be present in human milk and may reduce breast-milk production. The product information advises against Premarin use during breastfeeding; the need for treatment should be discussed with a healthcare professional.
Food, drink and interactions
The product information does not identify a specific direct Premarin-alcohol interaction, but excessive alcohol intake is listed as a factor that can increase stroke risk. Avoid excessive alcohol and discuss alcohol use with the prescriber if cardiovascular or other risk factors are present.
Food and drink
Premarin can be taken without regard to meals. Grapefruit juice may alter estrogen metabolism and may increase estrogen exposure, so discuss grapefruit consumption with the prescriber or pharmacist.
Storage
Store below 30°C and keep out of the sight and reach of children.
Frequently asked questions
How quickly might Premarin start helping hot flushes?
Clinical studies of Premarin 0.3 mg showed improvement in moderate to severe hot flushes by the first assessed time point at 4 weeks, with continued improvement at 12 weeks. Individual response varies, so treatment should be reviewed with the prescriber.
Is Premarin 0.3 mg the same as an estradiol tablet or patch?
No. Premarin contains a mixture of conjugated estrogens, while estradiol products contain estradiol. They are different formulations and their milligram strengths should not be treated as directly interchangeable.
Do I need progesterone with Premarin if I still have my uterus?
A progestogen should generally be considered when systemic estrogen is prescribed to a postmenopausal woman with an intact uterus. This reduces the increased risk of endometrial hyperplasia and cancer associated with unopposed estrogen.
Can Premarin 0.3 mg cause spotting or vaginal bleeding?
Yes. Breakthrough bleeding or spotting can occur, particularly during the first months of HRT. Bleeding that persists, begins after treatment has been established, or occurs unexpectedly after menopause should be assessed by a healthcare professional.
Can I take Premarin with grapefruit juice?
Grapefruit juice may affect how estrogen is metabolized and can increase estrogen exposure. Discuss regular grapefruit consumption with your prescriber or pharmacist.
What should I do if I forget a Premarin tablet?
Take the missed dose when you remember and then continue as directed. Do not take a double dose to make up for a forgotten tablet; missed tablets can cause breakthrough bleeding in women with a uterus.
Does Premarin increase the risk of blood clots?
Estrogen HRT is associated with an increased risk of venous blood clots. Seek urgent medical attention for painful swelling or redness of a leg, sudden chest pain, or difficulty breathing.
Is Premarin 0.3 mg made from the same estrogen as standard estradiol products?
No. Premarin contains conjugated estrogens obtained from natural sources and includes a mixture of estrogen sulfates; it is not simply an estradiol tablet.
Is Premarin 0.3 mg a prescription medicine?
Yes. Premarin is a prescription medicine. Prescription and import requirements can differ between countries.
