Medicine overview
Oestrogel is an oestrogen-only hormone replacement therapy containing estradiol, a form of the main oestrogen produced by the ovaries before menopause. The 0.06% transdermal gel is absorbed through the skin and is used mainly to relieve symptoms caused by falling oestrogen levels after menopause. Two pump actuations deliver 2.5 g of gel containing 1.5 mg estradiol. It may also be used to help prevent postmenopausal osteoporosis in selected women at high fracture risk.
What it is used for
Oestrogel is used as HRT for postmenopausal symptoms associated with oestrogen deficiency, including vasomotor symptoms such as hot flushes and sweating. It is also authorised for preventing osteoporosis in postmenopausal women who have a high future fracture risk and cannot tolerate, or have contraindications to, other medicines approved for osteoporosis prevention. Experience in women older than 65 years is limited.
How it works
Estradiol replaces some of the oestrogen that declines during and after menopause. Oestrogel delivers estradiol through the skin into the circulation, avoiding first-pass metabolism through the liver. Restoring oestrogen activity can reduce menopausal symptoms linked to oestrogen deficiency. Oestrogen also helps limit the increased bone turnover and loss of bone mineral that occur after menopause, which explains its role in osteoporosis prevention for appropriately selected women.
How it is taken
Oestrogel is for external skin use only and must not be swallowed. Apply the prescribed amount to clean, dry, intact skin on recommended areas such as the arms and shoulders or inner thighs, spreading it in a thin layer over a large area. Do not apply it on or near the breasts or to the vulval area. Allow the gel to dry for about 5 minutes before dressing, wash your hands with soap and water afterward, and avoid washing the application site, applying other skin products there, or allowing skin-to-skin contact with others for at least one hour. A new pump must be primed and the first dispensed dose discarded.
Important warnings
Do not use Oestrogel with known or suspected breast cancer, known or suspected oestrogen-dependent cancer, unexplained genital bleeding, untreated endometrial hyperplasia, previous or current venous thromboembolism, known thrombophilic disorders, active or recent arterial thromboembolic disease, acute liver disease or unresolved abnormal liver function following liver disease, porphyria, or hypersensitivity to its ingredients. Women with a uterus generally require consideration of a progestogen to reduce the endometrial hyperplasia and cancer risk associated with prolonged unopposed oestrogen. HRT is associated with important risks including venous thromboembolism, stroke and, depending on treatment type and duration, breast and ovarian cancer. Seek prompt medical attention for painful leg swelling, sudden chest pain or difficulty breathing. Treatment requires review if jaundice, deterioration in liver function, a significant rise in blood pressure, a new migraine-type headache, or pregnancy occurs. Estradiol can transfer to other people through skin contact; children in particular should not touch the application area. The gel contains ethanol and is flammable until dry. Important medicine interactions include enzyme-inducing anticonvulsants, rifampicin/rifabutin, some HIV medicines, St John's wort and lamotrigine.
Side effects and monitoring
Common reported effects include headache, nausea, abdominal pain, breast swelling or pain, breast enlargement, changes in menstrual or vaginal bleeding, vaginal discharge, endometrial hyperplasia, weight changes and fluid retention with peripheral oedema. Less common or serious effects include migraine, mood changes, vomiting, itching, venous thromboembolism, liver problems, jaundice and severe allergic reactions. Urgent medical assessment is needed for symptoms suggesting a blood clot, such as painful leg swelling, sudden chest pain or shortness of breath, or for jaundice or a severe allergic reaction. Periodic HRT reviews are recommended, with breast assessment and appropriate screening according to clinical circumstances.
Pregnancy
Oestrogel is not indicated during pregnancy. Pregnancy should be excluded before starting HRT, and treatment should be stopped if pregnancy occurs.
Breastfeeding
Oestrogel is not indicated during breastfeeding.
Food, drink and interactions
Alcohol can be consumed while using oestrogen HRT. However, reducing alcohol intake may help some menopausal symptoms, including hot flushes and sleep problems. The Oestrogel formulation itself contains ethanol and is flammable until the applied gel has dried.
Food and drink
No specific food restrictions are required with oestrogen gel; normal food and drink can be continued. Because Oestrogel is absorbed through the skin rather than taken orally, its administration does not depend on meals.
Storage
