How it works
Zoledronic acid is a bisphosphonate that binds strongly to mineralized bone and inhibits osteoclasts, the cells responsible for breaking down bone tissue. By reducing osteoclast activity, it slows excessive bone resorption. This action can lower calcium released from bone in cancer-related hypercalcemia and helps reduce skeletal complications when cancer or multiple myeloma affects bone.
How it is taken
Zoltero 4 is given into a vein as an intravenous infusion by a healthcare professional and should not be self-administered. The 4 mg/5 mL product is a concentrate for infusion. Zoledronic acid 4 mg infusions must be administered over no less than 15 minutes; kidney function, hydration and the treatment indication affect how therapy is managed. Calcium-containing infusion solutions must not be mixed with zoledronic acid.
Important warnings
Do not use zoledronic acid in patients with known hypersensitivity to the medicine or its components. Kidney injury, including renal failure, can occur; renal function should be assessed before treatment and serum creatinine monitored before each dose. Adequate hydration is important, particularly when treating hypercalcemia of malignancy. Hypocalcemia and other electrolyte disturbances can occur and should be corrected or monitored as appropriate. Osteonecrosis of the jaw has been reported, so a preventive dental examination is recommended before treatment and invasive dental procedures should generally be avoided during therapy where possible. Severe bone, joint or muscle pain and atypical femoral fractures have also been reported. Concurrent medicines that can impair kidney function, loop diuretics, aminoglycosides and other medicines that lower calcium require particular caution. Patients should not receive another zoledronic-acid product at the same time. Seek prompt medical attention for symptoms of a severe allergic reaction, marked muscle spasms or tingling that may indicate low calcium, new severe thigh or groin pain, or persistent jaw pain, swelling or poor healing.
Side effects and monitoring
Common reported effects include nausea, fatigue, anemia, bone pain, constipation, fever, vomiting and shortness of breath. An acute-phase reaction may develop within the first three days after an infusion, with fever, chills, fatigue, muscle or joint aches, bone pain and flu-like symptoms; these usually settle within several days. Serious or less common problems include kidney impairment or renal failure, hypocalcemia and other electrolyte abnormalities, osteonecrosis of the jaw, atypical femoral fractures, severe musculoskeletal pain, eye inflammation and severe hypersensitivity reactions. Serum creatinine should be checked before each dose, and calcium, phosphate, magnesium and hydration status may require monitoring. Prompt medical assessment is needed for severe allergic symptoms, reduced urine output, significant muscle spasms or tingling, eye pain or vision changes, persistent jaw symptoms, or unusual thigh or groin pain.
Pregnancy
Zoledronic acid can cause fetal harm based on its mechanism of action and animal data. Pregnancy status should be checked before starting treatment in females of reproductive potential, and effective contraception is advised during and after treatment.
Breastfeeding
Breastfeeding is not advised during and after zoledronic acid treatment. It is not known whether zoledronic acid passes into human milk, but the medicine can remain bound to bone for a prolonged period and may be released over time.
Food and drink
There is no food-timing requirement because Zoltero 4 is administered intravenously rather than swallowed. Adequate hydration is important, particularly when zoledronic acid is being used for hypercalcemia of malignancy.

Will I feel flu-like after my first Zoltero 4 infusion?
