Medicine overview

Zobone 4 Mg Injection contains zoledronic acid, a bisphosphonate medicine that reduces excessive breakdown of bone. The verified 4 mg zoledronic acid formulation is used intravenously for cancer-related high blood calcium and for skeletal complications associated with cancers that have spread to bone or multiple myeloma. Treatment is given by a healthcare professional as an intravenous infusion rather than self-injected. Kidney function, hydration and mineral levels require attention during treatment.

What it is used for

Zobone 4 Mg Injection is used to treat tumour-induced hypercalcaemia, where cancer causes calcium levels in the blood to become abnormally high. Zoledronic acid 4 mg is also used with appropriate anticancer treatment to reduce or delay skeletal complications in adults with advanced cancers involving bone, including breast, prostate and lung cancers and multiple myeloma.

How it works

Zoledronic acid belongs to the bisphosphonate class. It has a strong affinity for mineralised bone and inhibits osteoclasts, the cells responsible for breaking down old bone. Reducing osteoclast activity slows the release of calcium from bone and decreases excessive bone resorption. This action helps lower cancer-related high calcium levels and reduces bone complications associated with malignant disease involving the skeleton.

How it is taken

Zobone 4 Mg Injection is administered by intravenous infusion by a healthcare professional. A 4 mg zoledronic acid dose must be appropriately prepared/diluted and infused over no less than 15 minutes. Patients should be adequately hydrated before and after administration, while excessive hydration should be avoided in people at risk of heart failure. Kidney function should be assessed as clinically required, and reduced doses may be necessary for some patients with renal impairment. Do not self-administer this medicine.

Important warnings

Do not use zoledronic acid in patients with hypersensitivity to zoledronic acid, other bisphosphonates or formulation ingredients. Severe renal impairment is a major concern; dehydration, pre-existing kidney disease and other nephrotoxic medicines can increase the risk of renal deterioration or renal failure. Correct hypocalcaemia before treatment and monitor calcium, phosphate, magnesium and renal function as appropriate. Osteonecrosis of the jaw can occur, particularly in patients with cancer and dental risk factors; dental assessment and preventive care should be considered before treatment, and new jaw pain, swelling, loose teeth or non-healing mouth sores require prompt assessment. Serious hypocalcaemia, atypical femoral fractures, severe musculoskeletal pain and hypersensitivity reactions have also been reported. Use caution with aminoglycosides, calcitonin, loop diuretics, nephrotoxic medicines and anti-angiogenic medicines.

Side effects and monitoring

Commonly reported effects include an acute-phase or flu-like reaction within the first few days, with fever, fatigue, chills, headache, bone pain, muscle pain and joint pain. Nausea, vomiting, abdominal symptoms, constipation, dizziness, rash and changes in blood mineral levels may also occur. Serious or less common problems include renal impairment or acute renal failure, severe hypocalcaemia, osteonecrosis of the jaw, atypical femoral fracture, severe allergic reactions and inflammatory eye disorders. Seek medical attention for facial or throat swelling, breathing difficulty, severe weakness or muscle spasms, seizures, reduced urination, persistent jaw symptoms, or new unexplained thigh, hip or groin pain. Renal function and relevant mineral levels should be monitored during treatment.

Pregnancy

Zoledronic acid should not be used during pregnancy because it may harm the developing fetus. Women who could become pregnant should discuss pregnancy prevention with their healthcare professional.

Breastfeeding

Breastfeeding is not recommended during zoledronic acid treatment. It is not known whether zoledronic acid passes into human milk, and authoritative prescribing information advises against breastfeeding because of the potential for serious adverse effects in the breastfed child.

Food and drink

There is no oral food-timing requirement because this medicine is administered intravenously. Patients should be adequately hydrated before and after administration, subject to clinical considerations such as risk of cardiac failure.

Storage

Store at or below 25°C. Product-specific Sun Pharma prescribing information states not to refrigerate the undiluted concentrate. After dilution, the prepared infusion may be kept at 2°C to 8°C when necessary, but the total time from dilution through completion of administration must not exceed 24 hours; it should be brought to room temperature before administration. Keep out of reach of children.

Frequently asked questions

Will I feel unwell after my first zoledronic acid infusion?

Some people develop a short-term acute-phase reaction within three days of treatment, with symptoms such as fever, fatigue, chills, headache, muscle aches, joint pain or bone pain. These symptoms usually settle within a few days.

Should I see a dentist before receiving zoledronic acid?

A dental examination and appropriate preventive dental care are recommended before treatment when risk factors for osteonecrosis of the jaw are present. Tell your dentist that you receive zoledronic acid, particularly before an extraction or other invasive dental procedure.

What should I do if I develop jaw pain or a mouth sore that will not heal?

Contact your healthcare professional or dentist promptly. Jaw pain, swelling, loose teeth, discharge or sores that do not heal can be warning signs of osteonecrosis of the jaw and need assessment.

Why are kidney tests needed with zoledronic acid?

Zoledronic acid is eliminated through the kidneys and can cause deterioration in renal function. Serum creatinine should be assessed as appropriate during treatment, particularly before repeated oncology doses.

Do I need to be hydrated before the infusion?

Yes. Patients should have adequate hydration before and after zoledronic acid administration, although excessive hydration should be avoided in people at risk of heart failure.

Why does the dentist need to know that I am receiving Zobone?

Zoledronic acid is associated with osteonecrosis of the jaw, and invasive dental procedures such as tooth extraction can increase concern about this complication. Dental procedures during treatment should therefore be planned with the treating healthcare team when appropriate.

How long does the Zobone infusion take?
