For oncology zoledronic acid 4 mg formulations, administration is by intravenous infusion and the infusion must take at least 15 minutes. Kidney function should be assessed before treatment, and the prescribed amount may need adjustment in renal impairment. Zoledronic acid must not be mixed with calcium-containing or other divalent-cation-containing infusion solutions.

Important warnings

Do not use zoledronic acid in patients with hypersensitivity to zoledronic acid or the formulation components. Aminoglycosides, calcitonin and loop diuretics may increase the risk of low calcium, while nephrotoxic medicines can add to renal risk.

Side effects and monitoring

Common adverse effects reported with oncology zoledronic acid include nausea, fatigue, anemia, bone pain, constipation, fever, vomiting and shortness of breath. Serious or less common problems include renal deterioration or renal failure, severe hypocalcemia, osteonecrosis of the jaw, severe musculoskeletal pain, serious hypersensitivity reactions and atypical femur fractures. Seek medical attention for symptoms such as facial or throat swelling, breathing difficulty, persistent jaw pain or poor healing in the mouth, severe muscle spasms or tingling that may suggest low calcium, markedly reduced urination, or new unexplained thigh or groin pain. Serum creatinine should be checked before each dose; calcium, phosphate and magnesium may also require monitoring.

Pregnancy

Zoledronic acid can cause fetal harm and should not be used during pregnancy unless the treating specialist determines that its potential benefit justifies the risk. Pregnancy status should be checked before starting treatment in patients who can become pregnant, and effective contraception is advised during treatment.

Breastfeeding

Breastfeeding is not recommended during and after treatment with zoledronic acid. It is not known whether zoledronic acid is present in human milk, and the medicine can remain incorporated in bone for prolonged periods.

Food and drink
