Medicine overview

Zorrent 4 is a lyophilized intravenous medicine containing 4 mg of zoledronic acid. Zoledronic acid belongs to the bisphosphonate group of medicines, which reduce the breakdown of bone. It is used in adults with advanced cancers involving bone to help prevent skeletal complications and is also indicated for tumour-induced hypercalcaemia, where cancer causes an abnormally high calcium level in the blood. Zorrent 4 must be prescribed and administered by healthcare professionals experienced with intravenous bisphosphonates.

What it is used for

Zorrent 4 is used to prevent skeletal-related events in adults with advanced malignancies involving bone. These events can include pathological fractures, spinal compression, the need for radiation or surgery to bone, and tumour-induced hypercalcaemia. It is also used to treat adults who already have tumour-induced hypercalcaemia.

How it works

Zoledronic acid is a bisphosphonate with a strong affinity for mineralized bone. It mainly inhibits osteoclasts, the cells responsible for breaking down bone tissue. By reducing excessive bone resorption, it helps limit the bone destruction associated with advanced cancers involving bone. This action also reduces the release of calcium from bone and helps lower elevated blood calcium in tumour-induced hypercalcaemia.

How it is taken

Zorrent 4 is administered by intravenous infusion and should only be prepared and given by an appropriately experienced healthcare professional. The product contains a lyophilized medicine that requires preparation before infusion. The infusion should be given over no less than 15 minutes. Kidney function and serum creatinine should be assessed as clinically required, and reduced doses may be necessary in some patients with renal impairment. Patients should be appropriately hydrated while avoiding overhydration where clinically unsuitable.

Important warnings

Zorrent 4 requires particular caution in people with kidney impairment because zoledronic acid can cause deterioration in renal function, including acute renal failure. Serum creatinine should be monitored as directed, and dehydration and other medicines that can damage the kidneys may increase this risk. Hypocalcaemia must be corrected before treatment; calcium and vitamin D supplementation may be required. Osteonecrosis of the jaw has been reported, particularly in patients with cancer, so oral health and dental risk factors should be assessed and invasive dental procedures require careful clinical consideration. Report persistent jaw symptoms or loose teeth promptly. Severe allergic reactions, severe hypocalcaemia, atypical femoral fractures, and osteonecrosis at other sites have also been reported. Patients should report new thigh, hip, or groin pain. Caution is required with aminoglycosides, calcitonin, loop diuretics, potentially nephrotoxic medicines, thalidomide, and anti-angiogenic medicines because these combinations may increase specific risks.

Side effects and monitoring

Common effects include fever or flu-like symptoms, fatigue, chills, headache, nausea, vomiting, reduced appetite, diarrhoea, constipation, abdominal discomfort, bone pain, muscle pain, joint pain, anaemia, conjunctivitis, and changes in kidney function. Hypophosphataemia is very common, while hypocalcaemia and increased blood creatinine or urea can occur. Serious or less common problems include acute renal failure, severe hypocalcaemia, osteonecrosis of the jaw, severe hypersensitivity or anaphylaxis, atrial fibrillation, interstitial lung disease, and atypical femoral fractures. Seek medical attention for breathing difficulty, facial or throat swelling, severe weakness or muscle spasms, seizures, persistent jaw problems, or new unexplained thigh, hip, or groin pain. Kidney function, calcium, phosphate, magnesium, and other clinically relevant laboratory values may require monitoring.

Pregnancy

Zorrent 4 should not be used during pregnancy. Adequate human pregnancy data are unavailable, and animal studies have shown reproductive toxicity. Women who could become pregnant should be advised to avoid pregnancy during treatment.

Breastfeeding

Zorrent 4 is contraindicated during breastfeeding. It is not known whether zoledronic acid is excreted into human milk.

Storage

Store at a temperature not exceeding 30°C. Keep out of reach of children. Do not use after the expiry date.

Frequently asked questions

Will I feel flu-like symptoms after a zoledronic acid infusion?

This can happen. An acute-phase reaction with fever, fatigue, chills, bone pain, muscle pain, or joint pain is commonly reported within the first three days after zoledronic acid administration and usually resolves within a few days.

Why does my dentist need to know that I am receiving Zorrent 4?

Zoledronic acid is associated with osteonecrosis of the jaw, a condition in which jaw bone becomes damaged. Dental risk factors should be assessed, and your dentist and treating medical team should know about your treatment before invasive dental procedures are considered.

Tell your treating healthcare professional and dentist promptly about jaw pain, swelling, non-healing mouth sores, or loose teeth. These symptoms require assessment because osteonecrosis of the jaw is an identified risk of zoledronic acid treatment.

Why is kidney function checked before zoledronic acid treatment?

Zoledronic acid can impair kidney function and, in some cases, cause acute renal failure. Kidney function helps clinicians determine whether treatment is appropriate and whether dose modification or withholding treatment is necessary.

Does Zorrent 4 need to be infused slowly?

Yes. Zorrent 4 is administered intravenously, and zoledronic acid infusion should take no less than 15 minutes. Giving it appropriately is important because renal toxicity is a recognized risk.
