How it works

Zoledronic acid belongs to the bisphosphonate group of medicines. It binds strongly to mineralized bone and inhibits osteoclasts, the cells responsible for breaking down bone tissue. This reduces excessive bone resorption and limits the release of calcium from bone into the bloodstream. In cancer involving bone, slowing this process can help control malignancy-related high calcium levels and reduce complications caused by weakened or damaged bone.

How it is taken

Zoledronic acid 4 mg is administered into a vein by a trained healthcare professional. For the 4 mg/5 mL concentrate formulation, the required amount must be diluted before intravenous infusion and must not be given as a direct injection. The infusion should run for at least 15 minutes. Kidney function should be assessed before treatment, and adequate hydration is important. The exact dose and treatment interval depend on the indication and renal function and must be determined by the treating clinician.

Important warnings

Do not use zoledronic acid in patients with known hypersensitivity to the medicine or its components. It can cause clinically significant kidney injury, including renal failure; renal function should therefore be checked before treatment and monitored as appropriate. Hypocalcemia can occur and should be corrected before therapy. Osteonecrosis of the jaw has been reported, particularly in patients receiving treatment for cancer, so a dental assessment and appropriate preventive dental care should be considered before therapy. Avoid invasive dental procedures when possible during treatment. Severe bone, joint, or muscle pain and atypical femoral fractures have also been reported. Use caution with nephrotoxic medicines, aminoglycosides, loop diuretics, and other treatments that may lower calcium. Zoledronic acid products containing the same active ingredient should not be given concurrently.

Side effects and monitoring

Common adverse effects include nausea, fatigue, fever, vomiting, constipation, bone pain, anemia, and shortness of breath. Flu-like reactions such as fever, chills, muscle pain, or joint pain may occur after an infusion. Important serious effects include kidney impairment or renal failure, hypocalcemia, severe hypersensitivity reactions, osteonecrosis of the jaw, and atypical femoral fractures. Seek medical attention for facial or throat swelling, difficulty breathing, muscle spasms or tingling suggesting low calcium, reduced urination, persistent jaw pain or poor healing in the mouth, or new unexplained thigh or groin pain. Serum creatinine should be assessed before doses used for cancer-related indications, with calcium and other relevant electrolytes monitored as clinically appropriate.

Pregnancy

Zoledronic acid can cause fetal harm and should not be administered during pregnancy. Pregnancy status should be verified before starting treatment in females of reproductive potential, and effective contraception is advised during and after treatment because zoledronic acid can remain bound to bone for a prolonged period.

Breastfeeding

Breastfeeding is not recommended during and after treatment with zoledronic acid. It is not known whether zoledronic acid passes into human milk, and its prolonged retention in bone creates a potential risk of serious adverse effects in a breastfed child.

Food and drink

Zoledronic acid is given intravenously, so food does not determine its absorption. Adequate hydration is important, particularly around treatment, but excessive hydration should be avoided in patients in whom fluid overload may be a concern.
