Important warnings

Do not use zoledronic acid in patients with hypersensitivity to zoledronic acid or components of the injection. Dehydration and other medicines that can harm the kidneys can increase renal risk. Hypocalcemia must be corrected, and calcium, phosphate and magnesium may require monitoring. Severe bone, joint or muscle pain and atypical femur fractures can occur. Zoledronic acid products containing the same active ingredient should not be used concurrently.

Side effects and monitoring

Common adverse effects reported with zoledronic acid 4 mg in cancer-related treatment include nausea, fatigue, anemia, bone pain, constipation, fever, vomiting and shortness of breath. Serious or less common problems include kidney impairment or failure, hypocalcemia and other electrolyte disturbances, osteonecrosis of the jaw, severe musculoskeletal pain, atypical femur fractures, ocular inflammation and serious hypersensitivity reactions. Serum creatinine should be monitored, and calcium, phosphate and magnesium may also require assessment. Prompt medical attention is appropriate for breathing difficulty or swelling suggestive of an allergic reaction, severe or persistent jaw symptoms, new thigh or groin pain, significant muscle spasms or tingling, or severe eye pain or inflammation.

Pregnancy

Zoledronic acid can cause fetal harm based on its mechanism of action and animal data. Pregnancy status should be considered before treatment, and females of reproductive potential should be advised about effective contraception during and after treatment.

Breastfeeding

Breastfeeding is not advised during and after zoledronic acid treatment because of the potential for serious adverse reactions in a breastfed child. It is not known whether zoledronic acid is present in human milk.

Food and drink
