Acrotac 25 mg Capsule contains acitretin, an oral retinoid related to vitamin A. It is used for severe psoriasis in adults, particularly when the condition requires systemic treatment. Acitretin helps control abnormal growth and development of skin cells, which can reduce the thickening and scaling associated with psoriasis. Because it can cause serious adverse effects, especially severe birth defects if used during pregnancy, treatment requires careful medical supervision and regular monitoring.

What it is used for

Acitretin is indicated for severe psoriasis in adults. It may be used for difficult forms of psoriasis when systemic treatment is considered necessary. The medicine controls psoriasis rather than curing it, and symptoms can return after treatment is stopped. Its significant safety risks mean that it should be prescribed and monitored by a clinician experienced with systemic retinoid medicines.

How it works

Acitretin belongs to the retinoid group of medicines and is chemically related to vitamin A. Its exact mechanism in psoriasis is not fully established. Retinoids influence the growth, maturation, and differentiation of skin cells. These effects help reduce the excessive production and accumulation of skin cells that contribute to thick, scaly psoriatic plaques. Improvement may develop gradually, and the full clinical response can take several weeks or months.

How it is taken

Acrotac is taken by mouth. Acitretin capsules are generally taken with the main meal, exactly as prescribed. Swallow the capsule as directed and do not take more or less than the prescribed amount. Treatment and dose selection require medical supervision because individual response and adverse effects vary. Do not stop or alter treatment without consulting the prescriber.

Important warnings

Acitretin is highly teratogenic and can cause severe birth defects. It must not be used during pregnancy or by a person who could become pregnant during treatment or for at least 3 years after treatment unless the required pregnancy-prevention measures are followed. It is contraindicated in severe liver or kidney impairment, chronic abnormally high blood lipids, hypersensitivity to acitretin or other retinoids, and concurrent treatment with methotrexate or tetracycline antibiotics. Avoid vitamin A supplements and other retinoids unless specifically directed. Blood donation is prohibited during treatment and for at least 3 years afterward. Acitretin can cause liver injury, substantial increases in triglycerides and cholesterol, visual problems including reduced night vision, and increased intracranial pressure. Severe headache with nausea, vomiting, or visual disturbance requires prompt medical assessment.

Side effects and monitoring

Common effects include dry or cracked lips, dry mouth, dry or peeling skin, hair changes or hair loss, dry eyes, nosebleeds, and nail changes. More serious problems include liver injury, marked increases in blood lipids, pancreatitis associated with severe hypertriglyceridemia, severe skin reactions, visual changes, and symptoms of increased intracranial pressure. Seek medical attention promptly for yellow skin or eyes, dark urine, severe abdominal pain, persistent severe headache, significant vision changes, or a serious allergic reaction. Liver function and fasting blood lipids should be checked before treatment and periodically during therapy; additional monitoring may be required according to individual risk factors.

Pregnancy

Acitretin must not be used during pregnancy because it can cause severe fetal abnormalities. A person capable of becoming pregnant must avoid pregnancy during treatment and for at least 3 years after the final dose, with pregnancy testing and effective contraception as required by the applicable treatment program. Alcohol must also be avoided during treatment and for 2 months afterward in females capable of becoming pregnant because alcohol can promote formation of etretinate, which remains in the body much longer.

Breastfeeding

Do not breastfeed while taking acitretin. Acitretin can pass into breast milk and may harm a nursing infant.

Food, drink and interactions

Females capable of becoming pregnant must avoid alcoholic drinks, alcohol-containing foods, and medicines containing alcohol during acitretin treatment and for 2 months after stopping it. Alcohol can promote conversion of acitretin to etretinate, a retinoid that may remain in the body for a prolonged period and extend the risk to a future pregnancy.

Food and drink

Acitretin capsules should generally be taken with the main meal because food improves absorption. Females capable of becoming pregnant must avoid foods and drinks containing alcohol during treatment and for 2 months after the last dose.

Storage

Store at controlled room temperature, approximately 20°C to 25°C (68°F to 77°F), protected from light. Keep the container tightly closed and protect the medicine from excessive heat and humidity. Keep out of reach of children.

How long does acitretin take to work for psoriasis?

Improvement may take several weeks, and the full benefit can take 2–3 months or longer. Psoriasis can temporarily worsen during the first months of treatment, so discuss significant changes with your prescriber rather than changing treatment yourself.
