How it works

Side effects and monitoring
Common effects reported with sildenafil in adults treated for PAH include headache, flushing, indigestion, back pain, limb pain, muscle pain, and diarrhea. Edema has also occurred when sildenafil was used with intravenous epoprostenol. Serious or less common concerns include symptomatic low blood pressure, sudden vision loss, sudden decrease or loss of hearing, prolonged erection, allergic reactions, and pulmonary edema in patients with possible pulmonary veno-occlusive disease. Blood pressure should be monitored when sildenafil is used with other blood-pressure-lowering medicines. Sudden vision or hearing changes, fainting or severe hypotensive symptoms, breathing that rapidly worsens, or an erection lasting more than 4 hours requires prompt medical assessment.
Pregnancy
Available human data have not shown a clear association between sildenafil use during pregnancy and major birth defects, miscarriage, or adverse maternal or fetal outcomes, but the available evidence is limited. Untreated pulmonary arterial hypertension itself carries substantial maternal and fetal risks, including heart failure, stroke, premature delivery, and death. Treatment during pregnancy therefore requires specialist medical assessment rather than starting or stopping sildenafil without clinical supervision.
Breastfeeding
Sildenafil and its active metabolite have been detected in human breast milk. Available information is insufficient to determine the effects on a breastfed infant or on milk production. A clinician should assess breastfeeding and treatment in the context of the mother's need for PAH therapy.
Food, drink and interactions
