Medicine overview
Efil 5 mg Tablet contains tadalafil, a phosphodiesterase type 5 (PDE5) inhibitor. It is an oral medicine used in adult men for erectile dysfunction and for urinary symptoms associated with benign prostatic hyperplasia, an enlarged prostate. For erectile dysfunction, tadalafil improves the blood-flow response that occurs with sexual stimulation; it does not cause an erection without sexual arousal. The 5 mg strength can also be used in prescribed once-daily tadalafil regimens.
What it is used for
Efil 5 mg Tablet is used for erectile dysfunction, where a man has difficulty achieving or maintaining an erection suitable for sexual activity. Tadalafil 5 mg is also used for signs and symptoms of benign prostatic hyperplasia, such as urinary frequency, urgency, weak stream or difficulty emptying the bladder. It may be prescribed when ED and BPH occur together.
How it works
Tadalafil blocks phosphodiesterase type 5 (PDE5), an enzyme that normally breaks down cyclic guanosine monophosphate, or cGMP. During sexual stimulation, increased cGMP helps smooth muscle in penile blood vessels relax so more blood can enter the penis, supporting an erection. Tadalafil also acts on smooth muscle in the lower urinary tract and can improve urinary symptoms associated with an enlarged prostate. Sexual stimulation is still required for its erectile effect.
How it is taken
Take Efil 5 mg by mouth exactly as prescribed. Tadalafil tablets may be taken with or without food. When tadalafil 5 mg is prescribed as a once-daily regimen for BPH or ED with BPH, it is generally taken at approximately the same time each day. Do not take more than one tadalafil dose in a day unless the prescribing instructions specifically state otherwise, and do not combine it with another PDE5 inhibitor or ED medicine without medical advice.
Important warnings
Do not use tadalafil with nitrates such as nitroglycerin or isosorbide medicines, or with the guanylate cyclase stimulator riociguat, because blood pressure can fall dangerously. It should not be used for ED when sexual activity is medically inadvisable because of cardiovascular disease. Tell the prescriber about alpha-blockers, blood-pressure medicines, strong CYP3A4 inhibitors such as ketoconazole or ritonavir, kidney or liver disease, previous serious eye problems, bleeding disorders, and medicines for erectile dysfunction. Seek urgent medical care for an erection lasting more than 4 hours, sudden loss of vision, sudden decrease or loss of hearing, or chest pain, severe dizziness or nausea during sexual activity.
Side effects and monitoring
Common effects with tadalafil 5 mg include headache, indigestion, back pain, muscle aches, flushing, nasal congestion and, in some studies, diarrhea or dizziness. Less common but serious problems include prolonged erection or priapism, severe allergic reactions, sudden vision loss and sudden hearing loss. Seek urgent care for an erection lasting more than 4 hours, sudden visual or hearing changes, or cardiovascular symptoms during sex. Routine laboratory monitoring is not generally specified, but blood pressure and kidney, liver or cardiovascular factors may need clinical review in people with relevant conditions.
Pregnancy
Tadalafil tablets for ED/BPH are not indicated for use in females. There are no adequate human pregnancy data to establish drug-associated developmental risk.
Breastfeeding
Tadalafil tablets for ED/BPH are not indicated for use in females. It is not known whether tadalafil or its metabolites are present in human breast milk or what effects they could have on a breastfed child or milk production.
Food, drink and interactions
Large amounts of alcohol should be avoided or limited while taking tadalafil. Both alcohol and tadalafil can lower blood pressure, and substantial alcohol consumption can increase dizziness, headache, rapid heartbeat and postural hypotension or faintness when standing.

Tadalafil may be taken with or without food. Grapefruit juice can inhibit CYP3A4 and may increase tadalafil exposure, so patients who regularly consume grapefruit or grapefruit juice should discuss this with their prescriber or pharmacist.
