Medicine overview

Alivher 25 mg Tablet contains sildenafil citrate, providing 25 mg of sildenafil. Sildenafil belongs to the phosphodiesterase type 5 (PDE5) inhibitor class and is used for erectile dysfunction in adult men. It improves the natural erectile response by increasing blood flow into erectile tissue when sexual stimulation occurs. The medicine does not create sexual desire and does not produce an erection without sexual stimulation. It is taken by mouth and should be used according to the prescriber's instructions.

What it is used for

Alivher 25 mg Tablet is used for erectile dysfunction, a condition in which a man has difficulty getting or maintaining an erection adequate for sexual activity. Sildenafil supports the normal erectile response during sexual stimulation by improving penile blood flow. It does not cure the underlying cause of erectile dysfunction and does not increase sexual desire.

How it works

During sexual stimulation, nitric oxide is released in erectile tissue and increases levels of cyclic guanosine monophosphate, or cGMP. This chemical relaxes smooth muscle and allows more blood to enter the penis. Sildenafil blocks PDE5, the enzyme that normally breaks down cGMP. This allows the cGMP signal to last longer and supports an erection when sexual stimulation is present. Sildenafil has little erectile effect without sexual stimulation.

How it is taken

Take sildenafil exactly as prescribed. For erectile dysfunction, sildenafil tablets are generally taken as needed before anticipated sexual activity; official labeling allows administration about 30 minutes to 4 hours beforehand, with approximately 1 hour commonly used. Swallow the prescribed tablet dose with water. Do not use sildenafil more than once in a day unless specifically directed otherwise. It may be taken with or without food, although a high-fat or large meal can delay how quickly it starts working.

Important warnings

Do not use sildenafil with nitrates such as nitroglycerin, nitrate-containing recreational “poppers,” or guanylate cyclase stimulators such as riociguat because the combination can cause a dangerous fall in blood pressure. Tell the prescriber about alpha-blockers, blood-pressure medicines, ritonavir, strong CYP3A4 inhibitors such as ketoconazole or itraconazole, certain antibiotics, and any other erectile-dysfunction treatment. Sexual activity may pose additional risk for people with significant cardiovascular disease. Extra caution is required with low blood pressure, uncontrolled high blood pressure, recent heart attack or stroke, severe liver or kidney disease, previous NAION-related vision loss, penile deformity, or disorders that increase the risk of priapism. Seek urgent care for an erection lasting longer than 4 hours, sudden loss of vision, sudden reduction or loss of hearing, or serious chest symptoms during sexual activity.

Side effects and monitoring

Common effects include headache, flushing, indigestion, nasal congestion, dizziness, nausea, back or muscle pain, rash, and temporary visual changes such as blurred vision or altered colour perception. Serious or less common problems include severe hypotension, prolonged erection or priapism, sudden vision loss associated with NAION, sudden hearing loss, allergic reactions, fainting, and cardiovascular events. Obtain immediate medical help for an erection lasting more than 4 hours, sudden sight or hearing loss, severe allergic symptoms, fainting, or chest pain. Routine laboratory monitoring is not normally specified for intermittent erectile-dysfunction use, but cardiovascular and medication review may be appropriate before treatment.

Pregnancy

Sildenafil tablets for erectile dysfunction are not indicated for use in females. There are insufficient human pregnancy data to determine a drug-associated developmental risk. This product should not be used during pregnancy unless sildenafil has been specifically prescribed for another medically appropriate indication by a qualified clinician.

Breastfeeding

Sildenafil tablets for erectile dysfunction are not indicated for use in females. Limited data show that sildenafil and its active metabolite can pass into human milk, while effects on a breastfed infant and on milk production are not established.

Food, drink and interactions

Avoid excessive alcohol while using sildenafil. Heavy drinking can make it more difficult to achieve an erection and may add to dizziness or light-headedness in susceptible people. A controlled interaction study did not show additional blood-pressure lowering with a moderate tested alcohol exposure, but this does not make heavy drinking advisable.

Food and drink

Sildenafil may be taken with or without food. A large or high-fat meal can delay its onset of action. NHS guidance also advises avoiding grapefruit and grapefruit juice because they may increase the chance of sildenafil side effects.
