Medicine overview

Adequet 100 Tablet contains quetiapine fumarate and provides 100 mg of quetiapine. It is an oral atypical antipsychotic medicine used in the management of schizophrenia and bipolar disorder. Quetiapine changes the activity of several chemical messengers in the brain, particularly serotonin and dopamine. Adequet is the immediate-release tablet product; Abbott lists Adequet SR separately as a sustained-release formulation, so the two products should not be treated as interchangeable.

What it is used for

Quetiapine is used to treat schizophrenia and episodes associated with bipolar disorder, including manic and depressive episodes. Its exact approved uses can differ by country and formulation. Adequet 100 Tablet is the immediate-release 100 mg tablet and should be used only for the condition and regimen prescribed by the treating clinician.

How it works

Quetiapine is an atypical antipsychotic. Its therapeutic effects are thought to involve changes in serotonin and dopamine signaling in the brain, including antagonism at serotonin 5-HT2 and dopamine D2 receptors. These neurotransmitters help regulate mood, perception, thinking and behavior. By modifying their activity, quetiapine can help control symptoms associated with schizophrenia and bipolar disorder.

How it is taken

Adequet 100 Tablet is taken by mouth. Immediate-release quetiapine tablets may be taken with or without food and should be taken exactly as prescribed. Do not take more or less than prescribed or stop treatment suddenly without medical advice, because abrupt discontinuation can cause symptoms such as nausea, vomiting and insomnia. If treatment has been interrupted for a week or longer, contact the prescriber before restarting.

Important warnings

Quetiapine carries a warning for increased mortality in older adults with dementia-related psychosis and is not approved for treating dementia-related psychosis. Suicidal thoughts and behavior require particular attention in children, adolescents and young adults treated for depressive disorders. Other important risks include neuroleptic malignant syndrome, tardive dyskinesia, high blood glucose, abnormal lipid levels, weight gain, low blood pressure on standing, falls, low white blood-cell counts, seizures and QT prolongation. Strong CYP3A4 inhibitors such as ketoconazole or ritonavir and strong CYP3A4 inducers such as phenytoin, rifampin or St. John's wort can substantially alter quetiapine exposure. Seek urgent medical care for high fever with severe muscle stiffness or confusion, severe allergic reactions, seizures, fainting, uncontrolled movements, or suicidal thoughts or behavior.

Side effects and monitoring

Common effects include drowsiness, dizziness, dry mouth, constipation, nausea, increased appetite and weight gain. Serious effects can include neuroleptic malignant syndrome, tardive dyskinesia, seizures, severe allergic reactions, significant low blood pressure, abnormal heart rhythm, high blood glucose and severe reductions in white blood cells. Monitoring may include weight, blood glucose and lipid levels, with additional clinical or laboratory monitoring based on individual risk factors. Seek prompt medical attention for fever with muscle stiffness and confusion, uncontrolled movements, seizures, severe rash or swelling, difficulty breathing or swallowing, signs of infection, or marked symptoms of high blood sugar.

Pregnancy

Quetiapine should be used during pregnancy only after the benefits and risks have been assessed by a healthcare professional. Exposure to antipsychotic medicines including quetiapine during the third trimester may cause extrapyramidal symptoms, such as abnormal muscle movements, and/or withdrawal symptoms in the newborn. Do not stop prescribed quetiapine because of pregnancy without discussing treatment with the prescriber.

Breastfeeding

Quetiapine passes into human breast milk in small amounts; published data cited in prescribing information report a relative infant dose below 1% of the maternal weight-adjusted dose. The benefits of breastfeeding and the mother's clinical need for treatment should be considered together with possible effects on the breastfed infant. Discuss breastfeeding with the prescribing clinician rather than changing treatment independently.

Food, drink and interactions

Avoid alcohol while taking quetiapine. Alcohol can add to the drowsiness caused by quetiapine and may further impair alertness and coordination.

Food and drink

Immediate-release quetiapine tablets can be taken with or without food. Discuss grapefruit and grapefruit juice with the prescriber or pharmacist because grapefruit may affect medicines metabolized through CYP3A4. Maintain adequate fluid intake, particularly in circumstances that may increase the risk of dehydration or overheating.

Storage

Store at room temperature in the original, tightly closed container, protected from excess heat and moisture. Do not store in the bathroom. Keep the medicine out of the sight and reach of children.

Does quetiapine cause weight gain or make you feel hungrier?

It can. Increased appetite and weight gain are recognized effects of quetiapine, and weight should be monitored during treatment.

Why does quetiapine make me so sleepy?

Drowsiness and sedation are known effects of quetiapine. Avoid driving or operating machinery until you know how the medicine affects you.

Can I stop Adequet 100 Tablet if I feel better?

Does quetiapine require blood tests or other monitoring?

Monitoring may be needed during treatment. Clinicians may monitor weight, blood glucose and blood lipids, with other tests based on medical history and treatment response.

Is Adequet 100 the same as Adequet SR 100?

No. Abbott lists Adequet and Adequet SR as separate products; Adequet SR is the sustained-release product. Do not substitute one formulation for the other unless specifically instructed by the prescriber.
