Adalat Crono 60 mg is an oral modified-release tablet containing 60 mg of nifedipine. It belongs to the dihydropyridine calcium-channel blocker class and is used to manage high blood pressure and prevent chronic stable angina. How it works
Nifedipine blocks calcium entry through slow calcium channels in vascular smooth-muscle cells. This relaxes and widens arteries. In hypertension, the resulting vasodilation reduces peripheral vascular resistance and lowers blood pressure. In stable angina, nifedipine reduces vascular resistance and dilates coronary arteries, helping improve coronary blood flow while reducing the workload against which the heart pumps. The prolonged-release formulation is designed to provide sustained nifedipine delivery.
How it is taken
Adalat Crono is taken by mouth. Swallow the modified-release tablet whole with water; do not bite, chew, crush, or break it because this can interfere with controlled release. Nifedipine prolonged-release tablets may be taken with or without food. Follow the prescribed schedule and do not change the dose without medical advice.
Important warnings
Do not use nifedipine if you are allergic to nifedipine or the product ingredients. Prolonged-release Adalat formulations are contraindicated in cardiogenic shock, clinically significant aortic stenosis, unstable angina, and during or within one month after myocardial infarction; they are not intended to treat an acute angina attack. Caution is required with very low blood pressure, poor cardiac reserve, and medicines that affect CYP3A4 metabolism. Rifampicin can markedly reduce nifedipine exposure and should not be used concurrently with this formulation. CYP3A4 inhibitors, including some macrolide antibiotics, azole antifungals, and HIV protease inhibitors, can increase nifedipine concentrations. Other blood-pressure-lowering medicines can produce an additive fall in blood pressure. The modified-release tablet must remain intact. Seek urgent medical attention for severe fainting or hypotension, breathing difficulty or swelling suggesting a serious allergic reaction, or new/worsening chest pain.
Side effects and monitoring
Common effects include headache, peripheral oedema such as ankle or leg swelling, vasodilation, constipation, and feeling unwell. Less common effects include dizziness, flushing-related symptoms, palpitations, fast heartbeat, low blood pressure, fainting, nausea, abdominal discomfort, indigestion, and allergic reactions. Rare or serious reactions can include angioedema involving the throat, severe allergic reactions, significant hypotension, intestinal obstruction, jaundice, and severe skin reactions. Blood pressure should be monitored during treatment, particularly when nifedipine is used with other medicines that lower blood pressure or alter its metabolism.
Pregnancy
Nifedipine should not be used during pregnancy unless the woman's clinical condition requires treatment with it. Use requires an individual assessment of benefits and risks. Careful blood-pressure monitoring is particularly important when nifedipine is given with intravenous magnesium sulfate because an excessive fall in blood pressure can occur.
Breastfeeding
Nifedipine passes into breast milk, although available clinical evidence indicates that the breastfed infant receives only a small fraction of the maternal dose. Food and drink
The prolonged-release tablet can be taken with or without food. Avoid grapefruit and grapefruit juice while taking nifedipine.
