Medicine overview

Adalat LA 30 mg is a prolonged-release medicine containing nifedipine, a dihydropyridine calcium-channel blocker. It is used to treat high blood pressure and to help prevent chronic stable angina. The prolonged-release tablet is designed to release nifedipine gradually, allowing once-daily administration. Nifedipine relaxes arterial blood vessels, reducing resistance to blood flow and lowering blood pressure. The verified Adalat LA 30 mg product is a prolonged-release tablet, not an extended-release capsule.

What it is used for

Adalat LA 30 mg is indicated for treating all grades of hypertension (high blood pressure). It is also used to prevent chronic stable angina pectoris, either alone or together with a beta-blocker when clinically appropriate. It is intended for prevention of stable angina symptoms and should not be used to treat an acute angina attack.

How it works

Nifedipine blocks slow calcium channels, reducing the movement of calcium into vascular smooth-muscle cells. This relaxes arteries in the peripheral and coronary circulation. In hypertension, widening peripheral arteries reduces resistance against which the heart pumps and therefore lowers blood pressure. Relaxation of coronary arteries also contributes to nifedipine's anti-anginal effect. The prolonged-release formulation provides controlled delivery of nifedipine over the day.

How it is taken

Adalat LA is taken by mouth. The prolonged-release tablet should be swallowed whole with a glass of water and may be taken with or without food. It should be taken at approximately 24-hour intervals, preferably in the morning. Do not bite, chew, break, or crush the tablet because this can interfere with its prolonged-release properties. Avoid grapefruit and grapefruit juice while taking nifedipine. Follow the prescribed dose rather than changing treatment yourself.

Important warnings

Do not use Adalat LA in people with hypersensitivity to nifedipine, other dihydropyridines, or its ingredients. The product is contraindicated in cardiogenic shock, clinically significant aortic stenosis, unstable angina, during or within one month after myocardial infarction, and with rifampicin. It should not be used to treat an acute angina attack. The verified product information also lists important gastrointestinal contraindications, including previous gastrointestinal or oesophageal obstruction, reduced gastrointestinal lumen diameter, inflammatory bowel disease, and Crohn's disease. Caution is required with low blood pressure, poor cardiac reserve, diabetes, and medicines that inhibit or induce CYP3A4. Seek urgent medical care for severe allergic swelling, fainting, severe hypotension, new or worsening chest pain, or other severe reactions.

Side effects and monitoring

Common effects include headache, peripheral oedema or other swelling, vasodilation, constipation, and feeling unwell. Less common effects include dizziness, vertigo, palpitations, fast heartbeat, low blood pressure, fainting, nausea, indigestion, abdominal pain, nasal congestion, visual disturbances, and allergic reactions. Rare or serious reactions can include severe allergic swelling, anaphylactic reactions, intestinal obstruction, jaundice, serious skin reactions, and worsening angina. Blood pressure should be monitored, particularly when nifedipine is used with other blood-pressure-lowering medicines or medicines that alter nifedipine concentrations.

Pregnancy

Nifedipine should not be used during pregnancy unless the woman's clinical condition requires treatment with it. The product information recommends a careful individual assessment of benefits and risks because available evidence is insufficient to exclude adverse effects on the unborn or newborn child. Do not start, stop, or change prescribed nifedipine during pregnancy without medical guidance.

Breastfeeding

Nifedipine passes into breast milk, although available clinical evidence indicates that the breastfed infant receives only a small fraction of the maternal dose. Limited evidence has not identified specific safety concerns. The decision to breastfeed while using nifedipine should consider both the benefit of breastfeeding and the mother's need for treatment; medical advice is appropriate if the infant shows possible signs of low blood pressure.

Food, drink and interactions

Alcohol may increase nifedipine-related effects such as dizziness or low blood pressure. Avoiding alcohol is advisable, particularly if it makes you dizzy or light-headed. The product information also notes that alcohol can further affect the ability to drive or operate machinery.

Food and drink

Adalat LA may be taken with or without food. Do not consume grapefruit or grapefruit juice while taking nifedipine. Grapefruit inhibits CYP3A4 and can increase nifedipine concentrations, potentially increasing and prolonging its blood-pressure-lowering effect; the interaction may persist for at least three days after regular grapefruit-juice consumption.

Storage

Store in the original packaging to protect the medicine from light and do not store above 25°C. Keep out of the sight and reach of children.

Frequently asked questions

Is it normal to see what looks like an Adalat tablet in my stool?

Yes, this can occur with Adalat LA. The tablet's outer membrane does not digest completely, so what looks like a whole tablet may sometimes appear in the stool even though the medicine has been released.

Can I split or crush Adalat LA 30 mg if the tablet is difficult to swallow?

No. The prolonged-release tablet must be swallowed whole and should not be bitten, chewed, broken, or crushed because doing so can interfere with controlled release.

Can I drink grapefruit juice while taking nifedipine?

No. Grapefruit can increase nifedipine levels and strengthen or prolong its blood-pressure-lowering effect.

Why do my ankles swell after starting nifedipine?

Peripheral oedema, including ankle swelling, is a recognized common adverse effect of nifedipine. Discuss persistent or troublesome swelling with your healthcare professional, especially if it occurs with breathlessness or other new symptoms.

Can Adalat LA 30 mg be used when chest pain suddenly starts?

No. Adalat LA is used to prevent chronic stable angina and is specifically not intended to treat an acute angina attack. Follow the emergency treatment plan provided for sudden or worsening chest pain.

Can I take Adalat LA with other blood pressure medicines?

Sometimes, but the combination should be medically supervised. Other antihypertensive medicines can add to nifedipine's blood-pressure-lowering effect and may increase the risk of postural hypotension.

Is Adalat LA 30 mg a capsule?

No for the product verified here. Bayer's Adalat LA 30 mg is a pink, round prolonged-release tablet containing 30 mg nifedipine; an exact Bayer product matching the description “Adalat 30 mg Capsule ER” was not verified.

Do I need a prescription for Adalat LA 30 mg?

Yes. The verified UK Adalat LA 30 mg product is classified as a prescription-only medicine. Prescription requirements in other countries can differ.
