Medicine overview

Actorise 200 Injection contains darbepoetin alfa, an erythropoiesis-stimulating agent (ESA). It is supplied as a 200 mcg/0.4 mL prefilled syringe and is used to treat certain types of anaemia associated with chronic kidney disease or chemotherapy. Darbepoetin alfa acts like the natural hormone erythropoietin, signalling the bone marrow to produce more red blood cells. Actorise is a prescription medicine and should be administered according to medical supervision because treatment requires haemoglobin, blood pressure and other clinical monitoring.

What it is used for

Actorise 200 Injection is used to manage anaemia caused by chronic kidney disease. Darbepoetin alfa is also used for anaemia caused by myelosuppressive chemotherapy in selected patients with non-myeloid cancers. Erythropoiesis-stimulating agents are not substitutes for red blood cell transfusion when anaemia requires immediate correction. Their use in cancer also has important limitations because of cardiovascular, blood-clot and tumour-related risks.

How it works

Darbepoetin alfa is a laboratory-produced erythropoiesis-stimulating protein with effects similar to erythropoietin, a hormone normally produced mainly by the kidneys. It binds to erythropoietin receptors on precursor cells in the bone marrow and stimulates their development into red blood cells. Increasing red blood cell production can raise haemoglobin and reduce the need for red blood cell transfusions in appropriate patients. The response develops over time, so darbepoetin alfa is not suitable when anaemia needs immediate correction.

How it is taken

Actorise 200 Injection is given by subcutaneous injection, meaning under the skin, or intravenously into a vein as directed by the treating healthcare professional. The dose and interval depend on the indication, haemoglobin response and clinical circumstances. Haemoglobin should be monitored during treatment, and iron status should be assessed before and during therapy. Patients or caregivers should only administer darbepoetin alfa themselves if specifically instructed and trained by a healthcare professional.

Important warnings

Do not use darbepoetin alfa in patients with uncontrolled hypertension, pure red cell aplasia that developed following treatment with an erythropoietin protein, or a serious allergic reaction to darbepoetin alfa. Erythropoiesis-stimulating agents can increase the risk of death, myocardial infarction, stroke, venous thromboembolism and thrombosis of vascular access. In patients with cancer, ESAs have also been associated with shortened survival and/or increased tumour progression or recurrence in some clinical settings. Blood pressure should be controlled before and during therapy. Seizures, severe allergic reactions, severe skin reactions and loss of haemoglobin response require prompt medical assessment. Iron deficiency, bleeding, infection and inflammation should be investigated when response is inadequate.

Side effects and monitoring

Common adverse effects reported with darbepoetin alfa in patients with chronic kidney disease include high blood pressure, shortness of breath, peripheral swelling, cough and low blood pressure associated with procedures such as dialysis. In patients receiving chemotherapy, abdominal pain, swelling and thromboembolic events have been reported. Serious risks include heart attack, stroke, blood clots, seizures, severe hypertension, serious allergic reactions, severe skin reactions and pure red cell aplasia. Seek urgent medical attention for chest pain, sudden breathing difficulty, symptoms of stroke, seizures, severe allergic symptoms or signs of a blood clot. Haemoglobin and blood pressure require regular monitoring; iron status should also be checked before and during treatment.

Pregnancy

Human pregnancy data for darbepoetin alfa are limited and are insufficient to establish the risk of major birth defects or miscarriage. Animal studies identified adverse developmental findings at some exposures. The potential benefits and risks to the pregnant patient and fetus should be considered before treatment.

Breastfeeding

It is not known whether darbepoetin alfa is present in human breast milk, affects a breastfed child, or affects milk production. The benefits of breastfeeding should be considered together with the mother's clinical need for treatment and any potential risks to the child.

Storage

Store Actorise 200 Injection refrigerated at 2°C to 8°C and protect it from light. Do not freeze or shake the syringe. Keep it out of the reach of children.

Frequently asked questions

How worried should I be about blood clots or stroke while receiving darbepoetin alfa?

Darbepoetin alfa can increase the risk of serious cardiovascular and thromboembolic events, including stroke and blood clots. Treatment therefore requires careful haemoglobin and blood-pressure monitoring, and the lowest effective ESA dose is generally used to reduce transfusion requirements.

Why does my doctor check my haemoglobin regularly while I am receiving Actorise?

Haemoglobin monitoring shows how your body is responding and helps guide safe treatment. Raising haemoglobin too rapidly or to unnecessarily high levels with an ESA can increase cardiovascular and thromboembolic risks.

Do I still need my iron levels checked if I am receiving darbepoetin alfa?

Yes. Iron status should be evaluated before and during ESA treatment because adequate iron is needed to make red blood cells. Iron deficiency can contribute to a poor response to darbepoetin alfa.

Does Actorise work immediately when my haemoglobin is very low?

No. Darbepoetin alfa stimulates your body to produce red blood cells over time and is not a substitute for a red blood cell transfusion when anaemia requires immediate correction.

Can darbepoetin alfa raise blood pressure?

Yes. Hypertension is an important adverse effect of darbepoetin alfa. It should not be used when high blood pressure is uncontrolled, and blood pressure should be monitored during treatment.

Is Actorise a treatment for cancer itself?

No. Darbepoetin alfa does not treat cancer. It may be used in selected patients to manage anaemia caused by myelosuppressive chemotherapy, subject to important restrictions and safety considerations.
