Medicine overview
Actoforan 500 mg Injection contains pemetrexed, an antifolate antineoplastic medicine used in chemotherapy. It is supplied as a 500 mg lyophilized dry powder in a single-use vial and is prepared for intravenous infusion. Pemetrexed is used in specific treatment settings for non-squamous non-small cell lung cancer and malignant pleural mesothelioma. It interferes with folate-dependent processes that cancer cells need to make DNA and RNA and continue dividing.
What it is used for
Pemetrexed is used for certain forms and stages of non-squamous non-small cell lung cancer, including initial combination treatment, maintenance treatment, and treatment after previous chemotherapy where indicated. It is also used with cisplatin for malignant pleural mesothelioma in patients whose disease is not suitable for curative surgery. The exact combination and treatment plan depend on the cancer type, previous therapy, kidney function, and other clinical factors.
How it works
Pemetrexed is a folate analog metabolic inhibitor, also described as an antifolate antineoplastic medicine. After entering cells, it inhibits folate-dependent enzymes involved in producing building blocks needed for DNA and RNA. This disrupts processes necessary for cell replication. Cancer cells that are actively dividing are therefore less able to reproduce and grow, which provides pemetrexed's antitumor effect.
How it is taken
Actoforan is prepared from its lyophilized powder and administered by intravenous infusion by a healthcare professional. Pemetrexed treatment generally requires folic acid and intramuscular vitamin B12 supplementation to reduce treatment-related blood and gastrointestinal toxicity, and corticosteroid premedication is commonly used to reduce skin reactions. Kidney function and blood cell counts must be assessed during treatment. The dose and treatment combination are determined by the oncology team; patients should not attempt to prepare or administer this medicine themselves.
Important warnings
Pemetrexed can cause severe bone-marrow suppression, including neutropenia, anemia and thrombocytopenia, with an increased risk of infection and bleeding. Folic acid and vitamin B12 supplementation is required to reduce hematologic and gastrointestinal toxicity. Severe renal toxicity can occur, and kidney function should be assessed before treatment; standard prescribing information does not provide a recommended dose when creatinine clearance is below 45 mL/min. Serious blistering or exfoliative skin reactions, interstitial pneumonitis and radiation-recall reactions have been reported. Pemetrexed is contraindicated in patients with a history of severe hypersensitivity to pemetrexed. In patients with mild-to-moderate renal impairment, ibuprofen can increase pemetrexed exposure and toxicity, so its use around treatment requires specific medical management. Seek prompt medical attention for fever or infection symptoms, unusual bleeding, markedly reduced urine output, new or worsening shortness of breath or persistent cough, or severe blistering/peeling skin.
Side effects and monitoring
Common adverse effects associated with pemetrexed-containing regimens include fatigue, nausea, reduced appetite, vomiting, diarrhea, constipation, rash and changes in blood cell counts. Serious effects can include severe neutropenia or other bone-marrow suppression, infection or sepsis, renal failure, severe skin reactions, interstitial pneumonitis and severe gastrointestinal toxicity. Complete blood counts and renal function, including creatinine clearance, require monitoring during treatment. Contact the oncology team promptly for fever, infection symptoms, unusual bruising or bleeding, severe diarrhea or vomiting, reduced urine output, breathing difficulty, persistent cough, or blistering/peeling skin.
Pregnancy
Pemetrexed can cause fetal harm. Pregnancy status should be checked before treatment in females of reproductive potential. Effective contraception is advised during treatment and for 6 months after the last dose for females of reproductive potential; males with female partners of reproductive potential are advised to use effective contraception during treatment and for 3 months after the last dose.
Breastfeeding
Breastfeeding is not recommended during pemetrexed treatment and for 1 week after the last dose because of the potential for serious adverse reactions in a breastfed infant. It is not known whether pemetrexed or its metabolites are present in human milk.
Food and drink
No product-specific food or drink restriction was verified. Pemetrexed is administered intravenously rather than taken by mouth. Patients should follow their oncology team's instructions about hydration, nutrition and any other medicines or supplements used during treatment.
Frequently asked questions
Why do I need folic acid and vitamin B12 with pemetrexed?
They are used to reduce the severity of pemetrexed-related blood and gastrointestinal toxicity. Take the supplements according to the oncology team's instructions and keep scheduled vitamin B12 injection appointments.
How tired might I feel after pemetrexed treatment?
Fatigue is a commonly reported adverse effect of pemetrexed-containing treatment. Its severity varies between patients, so persistent or troublesome fatigue should be discussed with the oncology team.
Can pemetrexed lower my white blood cell count?
Yes. Pemetrexed can cause neutropenia and other forms of bone-marrow suppression. Blood counts are therefore monitored, and fever or other signs of infection require prompt medical attention.
Why is kidney function checked before pemetrexed treatment?
Pemetrexed is primarily eliminated through the kidneys, and reduced kidney function can increase exposure to the medicine and the risk of toxicity. Creatinine clearance is therefore assessed as part of treatment monitoring.
Can I take ibuprofen while receiving pemetrexed?
Check with the oncology team first. Ibuprofen can increase pemetrexed exposure in patients with mild-to-moderate kidney impairment, and prescribing information specifies avoiding it around pemetrexed administration in this group.
Is pemetrexed always given with other cancer medicines?
No. Depending on the type and stage of cancer and the treatment setting, pemetrexed may be used with medicines such as cisplatin, carboplatin or pembrolizumab, or may be used as a single agent. The oncology team determines the appropriate regimen.
What symptoms need urgent attention during pemetrexed treatment?
Contact the treating team promptly for fever or infection symptoms, unusual bleeding, reduced urine output, new or worsening breathing difficulty, persistent cough, or severe blistering or peeling skin. These can indicate potentially serious treatment complications.
