How it works

Acyclovir is activated mainly inside herpes-virus-infected cells. Its active form inhibits viral DNA polymerase, an enzyme the virus needs to copy its genetic material, and also interrupts the growing viral DNA chain. This suppresses replication of herpes simplex virus types 1 and 2. The cream treats an active herpes infection locally but does not eliminate latent herpes virus from the body or permanently cure recurrent cold sores.

How it is taken

Acivir Cream is for topical use. Follow the directions provided with the prescribed product. General acyclovir 5% cream guidance recommends starting at the earliest signs of a cold sore, cleaning and drying the affected area, and applying enough cream to cover the lesion and its outer edge. Wash your hands before and after application and avoid unnecessary rubbing. Do not put the cream in the eyes or inside the mouth or nose.

Important warnings

Do not use acyclovir cream if you have a known allergy to acyclovir, valacyclovir, or any ingredient in the formulation. It is intended for external skin use and should not be applied to the eyes or inside the mouth or nose. Irritation or allergic contact sensitization can occur. Evidence for acyclovir cream in immunocompromised patients is limited. Rare postmarketing reactions include angioedema and anaphylaxis; urgent medical attention is needed for facial or throat swelling, breathing difficulty, or another severe allergic reaction.

Side effects and monitoring

Common or local effects include dry or cracked lips, peeling or flaky skin, dryness, burning, stinging, and itching at the application site. Contact dermatitis and eczema have also been reported. Rare but serious postmarketing reactions include angioedema and anaphylaxis. Stop using the cream and seek urgent medical attention for breathing difficulty, marked facial or throat swelling, or other signs of a severe allergic reaction. Routine laboratory monitoring is not generally described for topical acyclovir cream because systemic absorption is minimal.

Pregnancy

Topically applied acyclovir is minimally absorbed into the bloodstream. Published experience over several decades has not identified a drug-associated increase in major birth defects, miscarriage, or adverse maternal or fetal outcomes with topical acyclovir.
