Medicine overview

Acivir 400 DT is an oral antiviral medicine containing 400 mg of aciclovir in each dispersible tablet. Aciclovir is a nucleoside analogue that acts against herpes viruses. The medicine is used for herpes simplex infections, including genital herpes, and for varicella-zoster infections such as chickenpox and shingles. It may also be used to suppress recurrent herpes simplex infections or prevent them in certain immunocompromised patients. The tablet is dispersed in water before administration.

What it is used for

Acivir 400 DT is indicated for several infections caused by susceptible herpes viruses. These include herpes simplex infections of the skin and mucous membranes, initial and recurrent genital herpes, chickenpox and shingles. It can also be prescribed to suppress recurrent herpes simplex infections in immunocompetent patients or prevent herpes simplex infection in immunocompromised patients. The product prescribing information additionally includes management of specified severely immunocompromised patients.

How it works

Aciclovir becomes activated mainly inside herpes-virus-infected cells. Viral thymidine kinase first converts it to aciclovir monophosphate, after which cellular enzymes convert it to the active triphosphate form. This active form inhibits viral DNA polymerase and interferes with production of new viral DNA. As a result, replication of susceptible herpes viruses is reduced. Aciclovir controls viral replication but does not eliminate latent herpes virus from the body.

How it is taken

Take Acivir 400 DT by mouth exactly as prescribed. According to the manufacturer, the dispersible tablet should be dispersed in one teaspoonful (5 mL) of boiled and cooled water before administration. Treatment schedules vary substantially according to the infection, age, immune status and kidney function, so the prescribed regimen should be followed. Maintain adequate hydration, particularly with higher doses or if kidney function is reduced.

Important warnings

Do not use aciclovir if you have a known hypersensitivity to aciclovir, valaciclovir or an ingredient in the product. Aciclovir is eliminated through the kidneys, so people with renal impairment and older adults may require dose adjustment and closer observation. Adequate hydration is important because renal adverse effects can occur. Neurological reactions are more likely when drug concentrations become excessive, particularly with impaired kidney function. Medicines that affect renal tubular secretion, including probenecid and cimetidine, can increase aciclovir exposure; interactions have also been reported with mycophenolate mofetil and theophylline. Seek medical attention for severe allergic reactions, marked confusion, hallucinations, seizures, reduced consciousness or signs of kidney problems.

Side effects and monitoring

Common adverse effects reported with oral aciclovir include headache, dizziness, nausea, vomiting, diarrhoea, abdominal pain, itching, rash, fatigue and fever. Less common or serious reactions can include urticaria, hair loss, severe allergic reactions, shortness of breath, liver abnormalities, hepatitis, jaundice, kidney impairment and neurological effects such as confusion, hallucinations, tremor, seizures or reduced consciousness. Seek prompt medical attention for facial or throat swelling, difficulty breathing, severe neurological symptoms or substantially reduced urine output. Kidney function may need monitoring in patients at increased renal risk.

Pregnancy

Aciclovir should be used during pregnancy after consideration of the expected benefit and potential risk. Available post-marketing pregnancy registry data have not shown an increased overall rate of birth defects compared with the general population, but treatment during pregnancy should still be discussed with a healthcare professional.

Breastfeeding

Aciclovir passes into breast milk. Manufacturer prescribing information advises caution during breastfeeding. Specialist breastfeeding guidance considers aciclovir compatible with breastfeeding for full-term, healthy infants when clinically required, with observation of the infant for effects such as rash, vomiting, diarrhoea, drowsiness or irritability.
