Medicine overview

Accuzide Forte 20 mg/12.5 mg is a prescription combination medicine for treating high blood pressure. Each film-coated tablet contains quinapril 20 mg and hydrochlorothiazide 12.5 mg. Quinapril is an angiotensin-converting enzyme (ACE) inhibitor, while hydrochlorothiazide is a thiazide diuretic. The two medicines lower blood pressure in complementary ways: quinapril relaxes blood vessels, while hydrochlorothiazide increases the removal of sodium and water through the kidneys. The combination is intended for patients for whom treatment with both components is appropriate.

What it is used for

Accuzide Forte is used to treat hypertension (high blood pressure) when combined treatment with quinapril and a diuretic is considered appropriate. Lowering persistently elevated blood pressure helps reduce the strain placed on the heart and blood vessels. This fixed-strength tablet provides both an ACE inhibitor and a thiazide diuretic in one oral medicine.

How it works

Quinapril is converted in the body to quinaprilat, which inhibits angiotensin-converting enzyme. This reduces production of angiotensin II, a substance that normally narrows blood vessels, helping the vessels relax and blood pressure fall. Hydrochlorothiazide acts on the kidneys to increase the excretion of sodium, chloride and water. Its longer-term antihypertensive action also reduces peripheral vascular resistance. Together, these effects help control elevated blood pressure.

How it is taken

Accuzide Forte is taken by mouth according to the prescribed regimen. The 20 mg/12.5 mg tablet is a fixed-strength combination and should be used only when this strength is appropriate for the patient's established treatment. The official product information allows the tablet to be divided into equal doses. Do not independently change the prescribed dose or treatment. Patients with impaired kidney function require particular medical assessment because this combination is not recommended in severe renal impairment.

Important warnings

Do not use Accuzide Forte in patients with hypersensitivity to its active ingredients or excipients, previous ACE-inhibitor-associated angioedema, hereditary or idiopathic angioedema, anuria, severe renal impairment, or during the second or third trimester of pregnancy. Concomitant sacubitril/valsartan is contraindicated because of the risk of angioedema; appropriate separation between treatments is required. Aliskiren-containing medicines are contraindicated with Accuzide in patients with diabetes or specified renal impairment. Important risks include symptomatic hypotension, kidney dysfunction, changes in potassium, sodium and other electrolytes, persistent ACE-inhibitor cough, and severe angioedema. Seek urgent medical attention for swelling of the face, lips, tongue or throat, difficulty breathing or swallowing, sudden severe eye pain or reduced vision, or acute breathing difficulty. Hydrochlorothiazide has also been associated with photosensitivity and an increased cumulative-dose risk of non-melanoma skin cancer; suspicious new or changing skin lesions should be assessed. Important interactions include lithium, potassium supplements or potassium-containing salt substitutes, other medicines affecting the renin-angiotensin system, NSAIDs, certain diabetes medicines, and medicines that increase the risk of angioedema. Blood pressure, kidney function and electrolytes may require monitoring.

Side effects and monitoring

Commonly reported effects include headache, dizziness, dry persistent cough, tiredness, nausea or vomiting, abdominal discomfort, diarrhoea, indigestion, sleepiness and muscle pain. Clinically important effects can include excessive lowering of blood pressure, abnormal sodium or potassium levels, impaired kidney function, gout or increased uric acid, altered blood glucose, serious allergic reactions and angioedema. Rare or serious reactions include acute angle-closure glaucoma, pancreatitis, severe skin reactions, blood-cell abnormalities and acute respiratory distress. Urgent medical care is needed for facial or throat swelling, breathing difficulty, fainting, sudden eye pain or visual deterioration, or other severe reactions. Monitoring may include blood pressure, kidney function, serum electrolytes and, where relevant, blood glucose and uric acid.

Pregnancy

Accuzide Forte should not be started during pregnancy. ACE inhibitors are not recommended during the first trimester and are contraindicated during the second and third trimesters because exposure can cause fetal kidney impairment, oligohydramnios, impaired skull ossification and neonatal complications including hypotension, kidney failure and hyperkalaemia. If pregnancy occurs during treatment, prompt medical review and an appropriate alternative antihypertensive treatment are required.

Breastfeeding

Accuzide Forte is not recommended during breastfeeding, particularly when nursing a premature or very young infant. Quinapril appears in breast milk at very low concentrations, while hydrochlorothiazide is excreted into human milk in small amounts and high thiazide doses can reduce milk production. A healthcare professional should assess whether another treatment is more appropriate.

Food, drink and interactions

Alcohol can increase the orthostatic blood-pressure-lowering effect of Accuzide Forte, which may increase dizziness or light-headedness, especially when standing. Patients should use caution with alcohol and discuss alcohol consumption with their healthcare professional.

Accuzide Forte can generally be taken without regard to food. Avoid using potassium supplements or potassium-containing salt substitutes unless specifically advised by a healthcare professional because quinapril can increase blood potassium. Vomiting, diarrhoea, excessive sweating or inadequate fluid intake can contribute to dehydration and excessive blood-pressure reduction.

Can Accuzide Forte make me feel dizzy when I stand up?
