Medicine overview
Abretone 250 mg Tablets contain abiraterone acetate, an androgen biosynthesis inhibitor used in the treatment of metastatic prostate cancer. Abiraterone blocks CYP17, an enzyme needed for androgen production. Lowering these hormones can slow the growth of prostate cancer that depends on androgen signalling. Abiraterone acetate is used with prednisone, and patients who have not had bilateral orchiectomy generally continue treatment that suppresses testicular androgen production. The 250 mg strength is taken by mouth as part of a clinician-prescribed regimen.
What it is used for
Abiraterone acetate is used with prednisone for metastatic castration-resistant prostate cancer, meaning prostate cancer that has spread and continues to progress despite androgen-deprivation therapy. It is also indicated with prednisone for metastatic high-risk castration-sensitive prostate cancer. Treatment forms part of androgen-deprivation therapy; patients receiving a gonadotropin-releasing hormone (GnRH) analogue generally continue it unless they have previously undergone bilateral orchiectomy.
How it works
Abiraterone is converted from abiraterone acetate after administration and inhibits CYP17 (17α-hydroxylase/C17,20-lyase). This enzyme is involved in producing androgens such as testosterone in the testes, adrenal glands and prostate tumour tissue. Blocking CYP17 substantially reduces androgen production. Because androgen signalling can stimulate prostate cancer cells, lowering androgen availability helps suppress the growth and progression of androgen-sensitive prostate cancer.
How it is taken
Take abiraterone acetate exactly according to the prescribed regimen and together with the prescribed corticosteroid. Standard abiraterone acetate tablets are taken as a single daily dose on an empty stomach. Do not eat for at least 2 hours before or 1 hour after the dose because food can substantially increase drug exposure. Swallow the tablets whole with water; do not crush or chew them. If one daily dose is missed, take the usual prescribed dose the following day rather than taking an extra dose. Contact the treating clinician if more than one daily dose is missed.
Important warnings
Abiraterone can cause hypertension, low blood potassium and fluid retention because of mineralocorticoid excess; blood pressure, potassium and signs of fluid retention require monitoring, particularly in people with cardiovascular disease. Serious and sometimes fatal liver injury can occur, so liver function tests are required before and during treatment. Adrenocortical insufficiency can occur, particularly if corticosteroid treatment is interrupted or during infection or physiological stress. Abiraterone plus prednisone/prednisolone should not be combined with radium Ra 223 because increased fractures and mortality have been observed. Strong CYP3A4 inducers can reduce abiraterone exposure. Abiraterone can also affect medicines metabolised by CYP2D6 and CYP2C8. Severe hypoglycaemia has been reported in some patients with diabetes receiving certain glucose-lowering medicines. Seek prompt medical attention for severe weakness, fainting, irregular heartbeat, significant swelling or shortness of breath, or symptoms suggesting liver injury such as yellow skin or eyes or dark urine.
Side effects and monitoring
Common adverse effects reported with abiraterone-containing treatment include fatigue, joint discomfort or swelling, peripheral oedema, hot flushes, diarrhoea, vomiting, cough, high blood pressure, shortness of breath and urinary tract infection. Laboratory abnormalities can include low potassium, anaemia, increased liver enzymes, increased cholesterol or triglycerides and increased blood glucose. Serious risks include severe hepatotoxicity, marked hypokalaemia with abnormal heart rhythms, adrenocortical insufficiency, severe hypoglycaemia and cardiovascular complications related to hypertension or fluid retention. Blood pressure, serum potassium and fluid retention should be checked at least monthly. Liver tests are required before treatment, every 2 weeks for the first 3 months and monthly thereafter, with more intensive monitoring in certain patients with hepatic impairment or abnormal results.
Pregnancy
Abiraterone acetate can cause fetal harm and is not indicated for use in women. Women who are pregnant or may become pregnant should not handle broken, crushed or damaged abiraterone acetate tablets without protection such as gloves. Men with female partners of reproductive potential should use effective contraception during treatment and for 3 weeks after the final dose.
Breastfeeding
Abiraterone acetate is not indicated for use in women. Information on the presence of abiraterone in human milk, its effects on a breastfed infant, or its effects on milk production is not established.
Food and drink
Standard abiraterone acetate tablets should be taken on an empty stomach. Do not eat food for at least 2 hours before and 1 hour after taking the medicine. Food can markedly increase abiraterone exposure and may increase the risk of adverse reactions. Swallow the tablets whole with water.
Storage
Store abiraterone acetate tablets at controlled room temperature, generally 20°C to 25°C (68°F to 77°F), with permitted excursions from 15°C to 30°C (59°F to 86°F). Keep out of reach of children. Pregnant women or women who may be pregnant should use protection such as gloves when handling broken, crushed or damaged tablets.
