Medicine overview
Abamune L 600 mg/300 mg Tablet is a fixed-dose antiretroviral medicine containing abacavir 600 mg and lamivudine 300 mg. Both active ingredients are nucleoside reverse transcriptase inhibitors (NRTIs), medicines that interfere with HIV replication. It is used with other antiretroviral medicines to treat HIV-1 infection. Abamune L is an oral tablet manufactured by Cipla Ltd. The medicine controls HIV infection but does not cure HIV.

How it works
Abacavir and lamivudine are NRTIs. Inside cells, they are converted into active forms that compete with the natural building blocks HIV uses to make viral DNA. Their incorporation into viral DNA stops further DNA-chain formation and inhibits HIV reverse transcriptase, an enzyme needed for viral replication. This reduces the production of new HIV copies. The combination suppresses HIV but does not eliminate the virus from the body.
How it is taken
Take Abamune L by mouth as prescribed as part of a complete antiretroviral regimen. The abacavir 600 mg/lamivudine 300 mg fixed-dose combination can be taken with or without food. HLA-B*5701 screening should be performed before abacavir is started. Because the tablet contains fixed amounts of both medicines, it is not suitable when either component requires individual dose adjustment. If a dose is missed, take it when remembered unless otherwise instructed; do not double the next dose.
Important warnings
Abacavir can cause a serious and potentially fatal hypersensitivity reaction. HLA-B*5701-positive patients must not receive abacavir, and testing should be performed before treatment begins. Fever, rash, nausea, vomiting, diarrhea or abdominal pain, severe tiredness or aches, shortness of breath, cough, or sore throat can indicate hypersensitivity and require immediate medical assessment. If abacavir is stopped because hypersensitivity is suspected, it must never be restarted. Severe hepatitis B exacerbations can occur after lamivudine is discontinued in people with HIV/HBV coinfection, so liver function requires follow-up. Lactic acidosis and severe liver enlargement with fatty changes have been reported with nucleoside analogues. The fixed-dose combination is not recommended when renal function requires lamivudine dose adjustment and is contraindicated in moderate or severe hepatic impairment. Important interactions include methadone, sorbitol-containing medicines, and riociguat. Immune reconstitution inflammatory syndrome may occur after antiretroviral therapy begins.
Side effects and monitoring
Common reported effects include nausea, diarrhea, headache or migraine, dizziness, tiredness or weakness, trouble sleeping, depressed mood, and hypersensitivity reactions. Serious effects include potentially fatal abacavir hypersensitivity, lactic acidosis, severe liver problems, severe hepatitis B flares after stopping lamivudine in people with HBV/HIV coinfection, and severe allergic or skin reactions. Seek urgent medical attention for symptoms suggesting hypersensitivity, breathing difficulty, severe rash, marked weakness, or signs of liver injury such as jaundice. HIV viral load and other routine HIV treatment parameters should be monitored. HLA-B*5701 status must be established before starting abacavir; renal and liver function and hepatitis B status are also clinically relevant.
Pregnancy
Available pregnancy-registry data have not shown a higher overall rate of birth defects with abacavir or lamivudine than the reference background rate. Pregnancy should still be discussed with the clinician managing HIV treatment so that an appropriate antiretroviral regimen can be maintained. A pregnancy exposure registry is available for people exposed to antiretroviral medicines during pregnancy.
Breastfeeding
Abacavir and lamivudine are present in human milk. U.S. prescribing information instructs mothers with HIV-1 who are receiving abacavir/lamivudine not to breastfeed because of concerns including HIV transmission, development of viral resistance, and possible adverse effects in the infant. Food, drink and interactions
Alcohol can reduce the elimination of abacavir and increase abacavir exposure. Clinical pharmacokinetic data found an increase in abacavir exposure with ethanol, although this was not considered clinically significant in the studied setting. Patients should discuss alcohol use with their HIV care provider, particularly if they have liver disease or other relevant medical conditions.
Food and drink
