What it is used for
Assurans 20 Mg is used for pulmonary arterial hypertension (WHO Group I). Sildenafil lowers resistance in the pulmonary blood vessels and can improve exercise ability in people with PAH. Although sildenafil is also used in other strengths and formulations for erectile dysfunction, the Indian regulatory entry for the 20 mg tablet specifically identifies pulmonary arterial hypertension as its approved indication.
How it works
Sildenafil inhibits phosphodiesterase type 5 (PDE5), an enzyme that normally breaks down cyclic guanosine monophosphate (cGMP). Increasing cGMP causes smooth muscle in the pulmonary blood vessels to relax. This widens the vessels in the lungs and reduces resistance to blood flow through the pulmonary circulation. In people with pulmonary arterial hypertension, this vasodilating effect can lower pulmonary vascular resistance and make physical activity easier.
How it is taken
Take the tablet by mouth exactly as prescribed. For adults with PAH, authoritative sildenafil 20 mg labeling gives a usual oral dose of 20 mg three times daily; dose changes should be made only by the prescribing clinician. Sildenafil for PAH may be taken with or without food. Do not combine it with another sildenafil product or another PDE5 inhibitor unless specifically directed by the treating clinician.
Important warnings
Do not use sildenafil with nitrates such as nitroglycerin or isosorbide, or with riociguat, because the combination can cause a dangerous fall in blood pressure. It is also contraindicated in people with a known hypersensitivity to sildenafil. Extra caution is required with low blood pressure, antihypertensive medicines, fluid depletion, severe left-ventricular outflow obstruction, pulmonary veno-occlusive disease, bleeding disorders, active peptic ulceration, or sickle-cell disease-associated pulmonary hypertension. Strong CYP3A inhibitors can substantially increase sildenafil exposure. Avoid using another PDE5 inhibitor at the same time. Seek urgent medical care for sudden vision loss, sudden hearing loss, severe faintness, pulmonary edema symptoms, or an erection lasting more than four hours.
Side effects and monitoring
Common effects reported with sildenafil in PAH include headache, flushing, indigestion, back pain, muscle pain, limb pain and diarrhea; nosebleeds can also occur. Serious or less common problems include significant hypotension, sudden loss or reduction of vision, sudden hearing loss, priapism and, in susceptible patients, pulmonary edema or vaso-occlusive complications. Seek prompt medical attention for sudden vision or hearing changes, fainting, severe breathing deterioration, or a prolonged erection. Blood pressure may need monitoring when sildenafil is used with other blood-pressure-lowering medicines.
Pregnancy
Available human data have not shown a clear association between sildenafil use during pregnancy and major birth defects, miscarriage, or adverse maternal or fetal outcomes, but the evidence is limited. Pulmonary arterial hypertension itself carries substantial pregnancy risks, so treatment decisions during pregnancy require specialist medical assessment rather than stopping or changing sildenafil without medical advice.
Breastfeeding
Sildenafil and its active metabolite have been detected in human milk. Available information is insufficient to determine the effects on a breastfed infant or on milk production. A healthcare professional should assess the benefits and potential risks when sildenafil is required during breastfeeding.
Food and drink
Sildenafil used for pulmonary arterial hypertension may be taken with or without food. Follow the prescribing instructions consistently and do not change the treatment schedule based on meals without medical advice.
