Medicine overview

Adalat Crono 30 mg is a modified-release nifedipine tablet used to treat high blood pressure and chronic stable angina. Nifedipine belongs to the dihydropyridine calcium-channel blocker class. It relaxes arterial blood vessels, reducing resistance against which the heart pumps and improving blood flow. The modified-release formulation is designed to release nifedipine gradually, allowing prolonged action rather than the rapid release produced by immediate-release nifedipine formulations.

What it is used for

Adalat Crono 30 mg is used for the treatment of arterial hypertension, where lowering elevated blood pressure helps control the condition. It is also indicated for chronic stable angina pectoris, sometimes called effort angina. In this condition, nifedipine reduces the workload on the heart and improves coronary blood flow. The modified-release formulation is intended for ongoing treatment and is not suitable for treating a sudden acute angina attack.

How it works

Nifedipine blocks L-type calcium channels in vascular smooth muscle. Calcium entering these cells normally contributes to contraction of the arteries. By reducing calcium entry, nifedipine relaxes and widens arterial blood vessels. This lowers peripheral vascular resistance and therefore blood pressure. Dilatation of coronary arteries and reduced cardiac workload also help prevent symptoms of chronic stable angina. Adalat Crono uses a controlled-release system to deliver nifedipine gradually over an extended period.

How it is taken

Take Adalat Crono by mouth exactly as prescribed. Modified-release nifedipine tablets are generally swallowed whole with water and should not be bitten, chewed, crushed or broken because this can interfere with controlled release. They may be taken with or without food. Take the medicine consistently at approximately the same time each day. Avoid grapefruit and grapefruit juice during treatment. Do not use the tablet to treat a sudden angina attack.

Important warnings

Do not use Adalat Crono if you are allergic to nifedipine or other dihydropyridines. Modified-release nifedipine is contraindicated in conditions including cardiogenic shock and clinically significant aortic stenosis, and certain product information also contraindicates use with unstable angina or during or shortly after myocardial infarction. It must not be taken with rifampicin because rifampicin can substantially reduce nifedipine exposure. Nifedipine is affected by CYP3A4 inhibitors and inducers, including some macrolide antibiotics, azole antifungals, HIV protease inhibitors, phenytoin, carbamazepine and phenobarbital. Other blood-pressure medicines can increase its hypotensive effect. Extra caution is required with very low blood pressure, heart failure, impaired liver function and certain gastrointestinal narrowing or obstruction disorders. Seek urgent medical care for severe fainting, difficulty breathing, swelling of the face or throat, or new or worsening chest pain.

Side effects and monitoring

Common effects include headache, peripheral oedema or ankle swelling, flushing or other vasodilatation, constipation and feeling unwell. Dizziness, low blood pressure, fainting, palpitations, rapid heartbeat, nausea, indigestion, abdominal discomfort and visual disturbances occur less commonly. Serious reactions can include severe allergic reactions or angioedema, marked hypotension, worsening chest pain and severe skin reactions. Blood pressure should be monitored during treatment, particularly when interacting medicines are started or stopped. Liver function and clinical response may require closer observation in patients with hepatic impairment.

Pregnancy

Nifedipine should not be used during pregnancy unless the woman's clinical condition requires it. Available product information recommends a careful individual assessment of benefits and potential risks, particularly because available evidence cannot completely exclude effects on the fetus or newborn. Blood pressure requires careful monitoring when nifedipine is used with intravenous magnesium sulfate because an excessive fall in blood pressure may occur.

Alcohol may increase dizziness or other effects that impair driving or operating machinery, particularly when nifedipine treatment is started or changed. Use caution with alcohol and discuss alcohol intake with the prescribing healthcare professional if dizziness or low blood pressure occurs.

Adalat modified-release tablets can generally be taken with or without food. Avoid grapefruit and grapefruit juice because they inhibit CYP3A4 and can increase nifedipine concentrations, potentially strengthening and prolonging its blood-pressure-lowering effect. After regular grapefruit consumption, this interaction can persist for several days.

Storage

Keep the tablets in their original packaging to protect them from light and moisture. Nifedipine is light-sensitive, so remove a tablet from its blister only immediately before use and do not break the tablet. Do not store above 30°C where specified by the product information.
