Medicine overview

Acuitel 5 is a film-coated tablet containing quinapril hydrochloride equivalent to 5 mg quinapril. Quinapril belongs to the angiotensin-converting enzyme (ACE) inhibitor class. It is used to treat essential hypertension and may also be used as adjunctive treatment in cardiac failure. By reducing the formation of angiotensin II, a substance that narrows blood vessels, quinapril helps blood vessels relax and lowers resistance against which the heart pumps.

What it is used for

Acuitel is indicated for essential hypertension, meaning high blood pressure without a specific secondary cause. It is also indicated as adjunctive medication for cardiac failure alongside diuretics and, particularly in serious cardiac failure, digitalis. Treatment and dose selection require medical supervision, especially in people with impaired kidney function, fluid or salt depletion, or other conditions that increase the risk of excessive blood-pressure reduction.

How it works

Quinapril is converted in the body to quinaprilat, which inhibits angiotensin-converting enzyme (ACE). ACE normally helps produce angiotensin II, a substance that constricts blood vessels and promotes aldosterone secretion. Reducing angiotensin II allows blood vessels to relax and lowers peripheral vascular resistance. In hypertension this reduces blood pressure, while in cardiac failure the reduced resistance and cardiac workload can support the heart's pumping function.

How it is taken

Acuitel is taken by mouth and can be taken independently of meals. The prescribed daily amount may be taken as a single dose or divided into two doses, depending on the treatment plan. The 5 mg tablet can be divided into equal doses. Dose selection and treatment duration should be determined by the treating clinician, with particular care in older adults, people with kidney impairment, and patients at risk of low blood pressure.

Important warnings

Do not use Acuitel in people with hypersensitivity to quinapril or its ingredients, a history of angioedema associated with ACE inhibitors, or during pregnancy. Concomitant sacubitril/valsartan is contraindicated because of the increased risk of angioedema; a 36-hour separation is required when switching between these treatments. Aliskiren must not be combined with quinapril in patients with diabetes or relevant renal impairment. Quinapril can cause symptomatic hypotension, deterioration in kidney function, and increased potassium. NSAIDs, potassium supplements or potassium-sparing medicines, lithium, diuretics, and other medicines affecting the renin-angiotensin system require particular attention. Seek urgent medical care for swelling of the face, lips, tongue or throat, difficulty breathing or swallowing, or other signs of severe angioedema. Kidney function, blood pressure and electrolytes require appropriate monitoring.

Side effects and monitoring

Common effects reported with Acuitel include cough or irritative cough, headache, dizziness, fatigue or weakness, nausea, vomiting, diarrhoea, abdominal discomfort, indigestion, back or muscle pain, skin reactions, and excessive lowering of blood pressure, particularly in susceptible patients. Serious or less common problems include angioedema, severe hypotension or fainting, hyperkalaemia, acute deterioration in kidney function, severe skin reactions, blood-cell abnormalities, pancreatitis and liver injury. Urgent assessment is needed for facial or throat swelling, breathing difficulty, fainting, jaundice, or a severe skin reaction. Monitoring may include blood pressure, kidney function, serum creatinine and electrolytes, particularly potassium; blood counts may also be monitored in higher-risk patients.

Pregnancy

Acuitel is contraindicated during pregnancy. ACE inhibitors can cause fetal toxicity, particularly with exposure during the second and third trimesters, including impaired fetal kidney function, oligohydramnios, delayed skull ossification, neonatal hypotension, hyperkalaemia, renal failure and potentially death. If pregnancy occurs, quinapril treatment should be stopped promptly under medical supervision and an appropriate alternative considered.

Breastfeeding

Acuitel should not be used while breastfeeding premature infants or during the first weeks after delivery because potential cardiovascular and kidney effects in the infant cannot be excluded and clinical experience is limited. The product information reports only very low quinapril concentrations in human milk, but breastfeeding use still requires medical assessment.

Food, drink and interactions

Alcohol can increase the effects of alcohol itself and may enhance orthostatic hypotension, causing a greater fall in blood pressure when standing. Caution with alcohol is therefore advised while taking Acuitel, particularly if dizziness or light-headedness occurs.

Food and drink

Acuitel can be taken independently of meals; food does not materially affect quinapril absorption. High dietary salt intake may reduce its blood-pressure-lowering effect. Potassium-containing supplements or salt substitutes require medical advice because quinapril can increase blood potassium.

Storage

Keep in a dry place and do not store above 25°C. Keep out of the sight and reach of children. Do not use after the expiry date printed on the carton or blister.

Frequently asked questions

Can quinapril cause a dry cough?

Yes. A persistent, non-productive cough is a recognised effect of ACE inhibitors, including quinapril. If a cough develops and remains troublesome, discuss it with the prescribing clinician rather than changing treatment yourself.
