Medicine overview
Activelle 1 mg/0.5 mg is a continuous combined hormone replacement therapy containing estradiol 1 mg and norethisterone acetate 0.5 mg in each film-coated tablet. Estradiol replaces some of the oestrogen that falls after menopause, while norethisterone acetate is a progestagen that helps protect the lining of the uterus from the effects of oestrogen. Activelle is intended for postmenopausal women with an intact uterus whose last natural period was more than one year ago.
What it is used for
Activelle is used to relieve troublesome symptoms caused by oestrogen deficiency after menopause, including vasomotor symptoms such as hot flushes. It may also be used to prevent osteoporosis after menopause in women who have a high risk of fractures and cannot use, or cannot tolerate, other medicines approved for osteoporosis prevention. Experience with Activelle in women older than 65 years is limited.
How it works
Estradiol in Activelle is chemically and biologically identical to human estradiol and replaces some of the oestrogen no longer produced after menopause. This helps reduce menopausal symptoms and limits postmenopausal bone loss. Norethisterone acetate acts like the natural hormone progesterone. Because oestrogen can stimulate growth of the endometrium, or womb lining, adding a progestagen helps reduce the risk of endometrial hyperplasia associated with oestrogen given alone to women who still have a uterus.
How it is taken
Activelle is taken by mouth as continuous combined HRT. The authorised product information states that one tablet is taken once daily without interruption, preferably at the same time each day. If a tablet is forgotten, it should be taken as soon as possible within 12 hours; after more than 12 hours, the missed tablet should be discarded. Missing a tablet can increase breakthrough bleeding or spotting. Treatment should follow the prescriber's instructions, using the lowest effective dose for the shortest appropriate duration.
Important warnings
Do not use Activelle with known, previous or suspected breast cancer; known or suspected oestrogen-dependent cancer; unexplained genital bleeding; untreated endometrial hyperplasia; previous or current deep-vein thrombosis or pulmonary embolism; known thrombophilic disorders; current or previous arterial thromboembolic disease such as angina or myocardial infarction; active liver disease or previous liver disease while liver tests remain abnormal; porphyria; or hypersensitivity to its ingredients. Combined HRT is associated with increased risks including venous thromboembolism, breast cancer and ischaemic stroke, with risk affected by factors such as age and duration of treatment. Seek urgent medical assessment for painful swelling of a leg, sudden chest pain or shortness of breath. Treatment should be stopped and medical advice obtained if pregnancy occurs, jaundice or worsening liver function develops, blood pressure rises significantly, or a new migraine-type headache appears. Persistent, new or unexplained bleeding after the initial treatment period requires medical investigation. Activelle contains lactose. Important interactions include medicines that alter hormone metabolism, such as carbamazepine, phenytoin, phenobarbital, rifampicin, rifabutin, some HIV medicines and St John's wort; ketoconazole may increase hormone levels. Activelle may also reduce lamotrigine concentrations and potentially affect seizure control.
Side effects and monitoring
Common or frequently reported effects include vaginal bleeding or spotting, breast pain or tenderness, headache, migraine, nausea, abdominal discomfort or bloating, back pain, fluid retention, depression or worsening depression, genital candidiasis or vaginitis, and weight increase. Bleeding and breast tenderness were among the most frequently reported effects in clinical trials and often occurred during the early months of treatment. Less common or serious problems include venous thromboembolism, pulmonary embolism, severe allergic reactions, stroke, myocardial infarction, gallbladder disease and significant blood-pressure changes. Obtain urgent medical help for symptoms suggesting a blood clot or severe allergic reaction. Regular clinical review is recommended during HRT, including appropriate breast screening and investigation of persistent or newly developing vaginal bleeding.
Pregnancy
Activelle is not indicated during pregnancy. If pregnancy occurs while taking Activelle, treatment should be stopped immediately and medical advice obtained.
Breastfeeding
Activelle is not indicated during breastfeeding.
Storage
Do not store above 25°C. Do not refrigerate. Keep the container in the outer carton to protect the tablets from light.
Frequently asked questions
Is bleeding or spotting normal after starting Activelle?
Bleeding or spotting can occur during the first few months of treatment. In clinical studies, bleeding or spotting became less common with continued treatment. Bleeding that persists beyond the initial months, begins after you have been stable on treatment, or continues after treatment stops should be medically investigated.
How long can it take before Activelle starts helping menopausal symptoms?
Relief of menopausal symptoms can occur during the first few weeks of treatment, although individual response varies. Your treatment should be reviewed by your prescriber rather than changing the dose yourself.
Can Activelle cause weight gain?
Weight increase was reported as an adverse effect in clinical trials, and fluid retention can also occur. A change in weight does not by itself establish that Activelle is the cause, so persistent or concerning changes should be discussed with a healthcare professional.
Is Activelle a contraceptive?
No. Activelle is hormone replacement therapy for postmenopausal women and should not be relied upon for contraception. Its authorised indication is HRT for women more than one year after their last menstrual period, with osteoporosis prevention in selected women as an additional indication.
What should I do if I forget an Activelle tablet?
If you remember within 12 hours, take the missed tablet as soon as possible. If more than 12 hours have passed, discard that tablet and continue according to the prescribed schedule; a missed dose may make breakthrough bleeding or spotting more likely.
Why does Activelle contain both estradiol and norethisterone acetate?
