Medicine overview
5 Flucel 500 Injection is an anticancer medicine containing 500 mg of fluorouracil in 10 mL (50 mg/mL). Fluorouracil, often called 5-FU, is a nucleoside metabolic inhibitor and antimetabolite chemotherapy medicine. It interferes with processes cancer cells need to make new DNA and RNA and to continue dividing. This injectable medicine is used for several cancers, including colorectal, breast, gastric, and pancreatic adenocarcinoma. It is administered intravenously by healthcare professionals experienced in cancer chemotherapy.
What it is used for
Fluorouracil is used as part of cancer treatment and may be given alone or with other anticancer medicines. Established indications include adenocarcinoma of the colon and rectum, breast cancer, gastric adenocarcinoma, and pancreatic adenocarcinoma. The exact chemotherapy regimen depends on the cancer being treated and other clinical factors. 5 Flucel is not intended for self-treatment or self-injection.
How it works
Fluorouracil is converted inside cells into active metabolites that interfere with the production and function of DNA and RNA. One important effect is inhibition of thymidylate synthase, an enzyme needed to produce a building block of DNA. Other fluorouracil metabolites can become incorporated into RNA and DNA. These effects interfere with cell division and are particularly damaging to rapidly dividing cells, including cancer cells.
How it is taken
5 Flucel 500 Injection is administered intravenously by or under the supervision of a healthcare professional experienced in cytotoxic chemotherapy. Fluorouracil may be given as an intravenous injection or intravenous infusion depending on the prescribed cancer regimen. The dose and treatment schedule are individualized according to the cancer, treatment combination, clinical condition, and toxicity. Patients should not attempt to inject or adjust fluorouracil themselves.
Important warnings
Fluorouracil can cause severe or fatal toxicity in people with complete dihydropyrimidine dehydrogenase (DPD) deficiency. Current prescribing information recommends evaluation for relevant DPYD genetic variants before treatment unless immediate therapy is necessary. Major risks include severe bone-marrow suppression, infection, diarrhea, mucositis, hand-foot syndrome, cardiotoxicity including myocardial ischemia or arrhythmias, neurologic toxicity, and hyperammonemic encephalopathy. Warfarin given with fluorouracil can cause clinically important increases in INR and requires close monitoring. Patients should urgently report chest pain, severe or persistent diarrhea or vomiting, fever or signs of infection, severe mouth sores, unusual bleeding, marked confusion, problems with coordination, or other severe symptoms.
Side effects and monitoring
Common or clinically important adverse effects include nausea, vomiting, diarrhea, mouth inflammation or sores, reduced appetite, hair loss, and hand-foot syndrome, which can cause redness, pain, swelling, or peeling of the palms and soles. Serious effects include severe myelosuppression with neutropenia or thrombocytopenia, severe gastrointestinal toxicity, cardiotoxicity, neurologic toxicity, hyperammonemic encephalopathy, and severe toxicity associated with DPD deficiency. Blood counts should be monitored during treatment, with additional clinical or laboratory monitoring based on the regimen and adverse effects. Severe diarrhea, fever, infection symptoms, chest pain, confusion, significant bleeding, or severe mouth sores require prompt medical assessment.
Pregnancy
Fluorouracil can cause fetal harm when administered during pregnancy. Patients who are pregnant, could become pregnant, or are planning pregnancy should discuss the risks with their oncology team. Effective contraception is required during treatment and for the recommended period after the final dose according to current prescribing guidance.
Breastfeeding
Breastfeeding is not recommended during fluorouracil treatment because of the potential for serious adverse reactions in a breastfed child. Patients who are breastfeeding should discuss feeding options with their oncology team before treatment.
Food, drink and interactions
NOT VERIFIED
Food and drink
There is no relevant food-timing requirement for the intravenous administration of this product. Patients experiencing nausea, vomiting, diarrhea, mouth sores, or poor intake during chemotherapy should discuss nutrition and hydration with their oncology team rather than making treatment changes themselves.
Storage
Store below 25°C. Do not refrigerate or freeze, and keep the container protected in its outer carton. Product-specific handling and disposal must follow cytotoxic-medicine procedures.
Frequently asked questions
Will I always feel sick after fluorouracil treatment?
No. Nausea and vomiting can occur, but their severity varies between patients and treatment regimens. Tell your oncology team if nausea or vomiting is persistent, severe, or prevents you from drinking enough fluids.
Can fluorouracil cause diarrhea and mouth sores?
Yes. Diarrhea and mucositis, which means inflammation and sores inside the mouth or digestive tract, are recognized fluorouracil toxicities. Severe diarrhea or significant mouth sores should be reported promptly because treatment-related toxicity can become serious.
What is hand-foot syndrome with fluorouracil?
Hand-foot syndrome can cause redness, tenderness, swelling, peeling, or pain on the palms of the hands and soles of the feet. Tell your treatment team if these symptoms develop, particularly if they interfere with normal activities.
Why might I be tested for DPD deficiency before receiving 5-FU?
DPD is an enzyme that helps the body break down fluorouracil. People with complete DPD deficiency have a much greater risk of severe or potentially fatal fluorouracil toxicity, so current prescribing information recommends testing for relevant DPYD variants before treatment unless therapy must begin immediately.
Can I inject 5 Flucel 500 at home myself?
No. Fluorouracil injection is cytotoxic chemotherapy and should be prepared and administered by appropriately trained healthcare professionals. The method and duration of administration depend on the prescribed chemotherapy regimen.
When should I seek urgent medical help during fluorouracil treatment?
Contact your treatment team urgently for fever or signs of infection, severe diarrhea or vomiting, severe mouth sores, unusual bleeding, chest pain, marked confusion, problems with coordination, or other rapidly worsening symptoms. These can indicate serious fluorouracil toxicity.
