Actorise 25 Injection contains darbepoetin alfa 25 mcg in a 0.42 mL prefilled syringe. It is an erythropoiesis-stimulating agent (ESA), a medicine that encourages the bone marrow to make red blood cells. Darbepoetin alfa is used to manage certain types of anaemia associated with chronic kidney disease and chemotherapy. The medicine is administered by injection and requires medical supervision, appropriate haemoglobin monitoring, and careful management of cardiovascular and blood-clot risks.

What it is used for

Darbepoetin alfa is used to treat anaemia associated with chronic kidney disease. It is also used for anaemia caused by myelosuppressive chemotherapy in selected patients with non-myeloid cancers. Treatment is intended to stimulate red blood cell production and, in appropriate settings, reduce the need for red blood cell transfusions. Its use in cancer-related anaemia is subject to important restrictions because ESAs can increase serious cardiovascular, thromboembolic, and cancer-related risks.

How it works

Darbepoetin alfa acts like erythropoietin, a natural hormone produced mainly by the kidneys. Erythropoietin signals the bone marrow to produce red blood cells. Chronic kidney disease can reduce natural erythropoietin production, while chemotherapy can suppress red blood cell formation. Darbepoetin alfa binds to erythropoietin receptors on blood-forming cells and stimulates red blood cell production, gradually increasing haemoglobin when the patient responds to treatment.

How it is taken

Actorise 25 Injection is administered by injection. Darbepoetin alfa can be given subcutaneously, meaning under the skin, or intravenously, meaning into a vein, depending on the indication and clinical circumstances. The dose and treatment interval must be determined from the patient's condition, haemoglobin response and other clinical factors. Patients or caregivers should administer darbepoetin alfa themselves only when specifically instructed and trained by a healthcare professional.

Important warnings

Do not use darbepoetin alfa in patients with uncontrolled hypertension or in patients who have developed pure red cell aplasia following treatment with darbepoetin alfa or another erythropoietin protein. ESAs can increase the risks of death, myocardial infarction, stroke, venous thromboembolism and vascular-access thrombosis. In patients with cancer, ESAs may increase the risk of tumour progression or recurrence in some settings. Blood pressure should be controlled before and during treatment. Severe allergic reactions and serious skin reactions can occur. New or worsening seizures require medical assessment. If haemoglobin does not respond as expected, causes such as iron deficiency, bleeding, infection or pure red cell aplasia should be investigated.

Side effects and monitoring

Common or clinically important effects reported with darbepoetin alfa in chronic kidney disease include hypertension, shortness of breath, peripheral oedema, cough, low blood pressure associated with procedures, rash and vascular-access complications. Injection-site pain can also occur. Serious risks include blood clots, heart attack, stroke, seizures, severe hypertension, severe allergic reactions and pure red cell aplasia. Haemoglobin and blood pressure require regular monitoring; iron status should also be evaluated because adequate iron is needed for red blood cell production. Seek urgent medical attention for chest pain, sudden breathing difficulty, one-sided weakness, severe headache, seizures, symptoms of a blood clot, or a serious allergic reaction.

Pregnancy

Available human pregnancy data are insufficient to determine whether darbepoetin alfa causes major birth defects or miscarriage. Treatment during pregnancy requires consideration of the expected maternal benefit and possible fetal risks.

Breastfeeding

It is not known whether darbepoetin alfa is present in human milk, affects a breastfed child, or affects milk production. The benefits of breastfeeding should be considered together with the mother's clinical need for treatment and any potential risk to the infant.

Storage

Store refrigerated at 2°C to 8°C. Do not freeze or shake. Protect from light and keep the prefilled syringe in its original carton until use. A syringe that has been frozen or improperly handled should not be used.
