Abstet 250 mg Tablets contain abiraterone acetate, an androgen biosynthesis inhibitor used in the treatment of metastatic prostate cancer. Abiraterone blocks CYP17, an enzyme needed to produce androgens such as testosterone. Lowering these hormones can slow the growth of prostate cancer that depends on androgen signalling. Abstet is an oral prescription medicine and is used as part of a treatment regimen that includes a corticosteroid. What it is used for
Abstet is used as part of treatment for advanced prostate cancer. Abiraterone acetate is established for metastatic castration-resistant prostate cancer, where disease progresses despite androgen-deprivation therapy, and metastatic high-risk castration-sensitive prostate cancer. It is administered with a corticosteroid such as prednisone or prednisolone according to the prescribed treatment regimen. How it works
Abiraterone acetate is converted in the body to abiraterone, which inhibits CYP17, an enzyme involved in androgen production. Androgens such as testosterone can stimulate the growth of androgen-sensitive prostate cancer cells. CYP17 inhibition reduces androgen production in the testes, adrenal glands, and prostate tumour tissue. How it is taken
Take Abstet by mouth exactly as prescribed. Abiraterone acetate tablets should be taken on an empty stomach; do not eat for at least 2 hours before taking the medicine and for at least 1 hour afterward. Swallow the tablets whole with water and do not crush or chew them. Treatment is normally used with a prescribed corticosteroid, and patients without prior bilateral orchiectomy generally continue a GnRH analogue. Do not alter the prescribed dose or corticosteroid treatment without medical direction.
Important warnings
Abiraterone can cause high blood pressure, low blood potassium, and fluid retention; blood pressure, potassium, and signs of fluid retention require monitoring. Particular caution is needed in people with cardiovascular disease, including heart failure, recent myocardial infarction, or ventricular arrhythmias. Serious liver injury, including acute liver failure, has been reported, so liver function testing is required before and during treatment. Adrenal insufficiency can occur, particularly if corticosteroid treatment is interrupted or during infection or physiological stress. Abiraterone plus prednisone/prednisolone should not be combined with radium Ra 223 because increased fractures and mortality have been observed. Strong CYP3A4 inducers can reduce abiraterone exposure, while abiraterone can affect medicines metabolized by CYP2D6 and CYP2C8. Severe hypoglycemia has occurred in patients with diabetes taking certain antidiabetic medicines, including pioglitazone or repaglinide. Seek urgent medical care for severe breathing or swallowing difficulty, facial or throat swelling, severe weakness or fainting, symptoms of significant liver injury, or other severe reactions.
Side effects and monitoring
Common adverse effects include fatigue, joint pain, high blood pressure, nausea, swelling or fluid retention, low potassium, hot flushes, diarrhea, vomiting, cough, headache, and upper respiratory infections. Serious or less common problems include severe hypokalemia, cardiac complications, adrenal insufficiency, serious liver injury, severe hypoglycemia in susceptible patients, severe allergic reactions, and non-infectious pneumonitis. Monitoring includes blood pressure, serum potassium, fluid retention, and liver function tests; blood glucose should also be monitored appropriately in patients with diabetes. Prompt medical assessment is needed for marked swelling, muscle weakness, irregular heartbeat, fainting, jaundice, severe shortness of breath, or signs of an allergic reaction.
Pregnancy
Abiraterone is not indicated for use in women and can cause fetal harm. Women who are pregnant or may become pregnant should not handle the tablets without protective gloves. Men with female partners who can become pregnant should follow the contraception instructions provided with their treatment.
Breastfeeding
Abiraterone is not indicated for use in women. Because the product is intended for male patients with prostate cancer and adequate human lactation data are not established for this use, breastfeeding while taking Abstet is not appropriate.
Food and drink
Abstet must not be taken with food. Food can substantially increase abiraterone exposure and may increase the risk of adverse effects. Do not eat for at least 2 hours before taking the tablets or for at least 1 hour afterward. Swallow the tablets whole with water.
