Serious concerns include significant hypotension, sudden vision loss, sudden hearing loss, priapism and hypersensitivity reactions. Patients should receive medical attention promptly for sudden visual or hearing changes, fainting or marked symptoms of low blood pressure, severe allergic symptoms, or an erection that persists abnormally. Blood pressure and the patient's clinical response to PAH treatment should be assessed as appropriate.
Pregnancy
Limited human data have not identified a clear association between sildenafil use during pregnancy and major birth defects, miscarriage, or adverse maternal or fetal outcomes. However, pulmonary arterial hypertension itself carries substantial risks during pregnancy. Use during pregnancy requires specialist assessment of the potential benefits and risks.
Breastfeeding
Sildenafil and its active metabolite have been reported in human breast milk, but available data are too limited to determine the risk to a breastfed infant.
