Pulmoday 20 mg Tablet contains sildenafil citrate equivalent to 20 mg sildenafil. It is an oral phosphodiesterase type 5 (PDE5) inhibitor used for pulmonary arterial hypertension (PAH), a condition involving abnormally high pressure in the arteries supplying the lungs. By relaxing blood vessels in the lungs, sildenafil reduces pulmonary vascular resistance and helps blood move through the pulmonary circulation more easily. Pulmoday is indicated in adults with WHO Group I PAH to improve exercise ability and delay clinical worsening.

How it works

Sildenafil is a PDE5 inhibitor. PDE5 normally breaks down cyclic guanosine monophosphate (cGMP), a signalling substance involved in blood-vessel relaxation. By inhibiting PDE5, sildenafil increases the effect of the nitric oxide–cGMP pathway and promotes relaxation of vascular smooth muscle. In pulmonary arterial hypertension, this produces vasodilation in the pulmonary circulation, helping reduce resistance to blood flow through the lungs and supporting exercise capacity.

Pulmoday is taken by mouth. For sildenafil 20 mg tablets used for adult PAH, authoritative prescribing information specifies 20 mg three times daily; patients should take sildenafil exactly as prescribed and should not change the dose or stop treatment without speaking to their healthcare provider. If too much is taken, medical advice or emergency assessment should be obtained promptly.

Important warnings

Do not use sildenafil with organic nitrates such as nitroglycerin or isosorbide preparations, or with riociguat, because the combination can cause a dangerous fall in blood pressure. It must not be used in people with known hypersensitivity to sildenafil or tablet ingredients. Sildenafil can lower blood pressure, particularly in people who already have hypotension or take blood-pressure-lowering medicines. It is not recommended for pulmonary veno-occlusive disease because pulmonary edema may occur. Strong CYP3A inhibitors can substantially increase sildenafil exposure, while CYP3A inducers can reduce exposure. Do not combine it with Viagra or another PDE5 inhibitor. Side effects and monitoring

Common adverse effects reported with sildenafil treatment for PAH include headache, flushing, indigestion, diarrhea, back pain, muscle pain, pain in the arms or legs, and nosebleeds. Serious effects include clinically significant hypotension, sudden decrease or loss of vision, sudden hearing loss, and prolonged erection (priapism). Seek urgent medical attention for sudden vision or hearing changes or an erection lasting longer than four hours. Blood pressure may require monitoring, particularly when sildenafil is used with antihypertensive medicines.

Pregnancy

Limited human data have not identified a clear association between sildenafil use during pregnancy and major birth defects, miscarriage, or adverse maternal or fetal outcomes. However, pregnancy with untreated PAH carries substantial risks, including maternal heart failure, stroke, preterm delivery, and maternal and fetal death. Treatment during pregnancy therefore requires specialist medical management rather than stopping or changing sildenafil without medical advice.

Breastfeeding

Sildenafil and its active metabolite have been detected in human breast milk. Available information is insufficient to determine the effects on a breastfed infant or on milk production. A healthcare professional should assess breastfeeding and treatment considerations individually.

Food, drink and interactions

A controlled interaction study found that sildenafil 50 mg did not increase the blood-pressure-lowering effect of alcohol in healthy volunteers at the tested alcohol exposure. This does not establish the safety of alcohol specifically with Pulmoday 20 mg in patients with PAH, who may already be susceptible to low blood pressure or dizziness.
