Joshila 50 Mg Tablet contains sildenafil, a phosphodiesterase type 5 (PDE5) inhibitor used to treat erectile dysfunction in adult men. Sildenafil improves blood flow to the penis during sexual stimulation, helping an erection develop and be maintained. Joshila is an oral tablet containing 50 mg sildenafil. Sexual stimulation is still required for the medicine to work; sildenafil does not automatically cause an erection or increase sexual desire.

What it is used for

Joshila 50 Mg Tablet is used for the treatment of erectile dysfunction, which means difficulty getting or maintaining an erection firm enough for sexual activity. Sildenafil supports the natural erectile response by improving penile blood flow when sexual stimulation occurs. This product should not be assumed to have the same indications or dosing instructions as sildenafil products specifically formulated and approved for pulmonary arterial hypertension.

How it works

Sildenafil blocks phosphodiesterase type 5 (PDE5), an enzyme that breaks down cyclic guanosine monophosphate (cGMP). During sexual stimulation, nitric oxide increases cGMP in erectile tissue. Higher cGMP levels relax smooth muscle and allow more blood to enter the penis. By slowing the breakdown of cGMP, sildenafil strengthens this normal blood-flow response and can make it easier to achieve and maintain an erection.

How it is taken

Joshila 50 Mg Tablet is swallowed by mouth. For sildenafil used for erectile dysfunction, prescribing information states that it is generally taken as needed approximately 1 hour before sexual activity and may be taken 30 minutes to 4 hours beforehand. It should not be taken more than once in a 24-hour period. It can be taken with or without food, although a high-fat or heavy meal can delay its onset. Follow the prescribed instructions and do not change the dose without medical guidance.

Important warnings

Do not use sildenafil with nitrates or nitric oxide donors, including medicines such as nitroglycerin, because the combination can cause a dangerous fall in blood pressure. It is also contraindicated with guanylate cyclase stimulators such as riociguat. Tell the prescriber about heart disease, low blood pressure, recent heart attack or stroke, liver or kidney problems, bleeding disorders, penile deformity, conditions that increase the risk of priapism, and previous non-arteritic anterior ischemic optic neuropathy (NAION). Alpha-blockers and other blood-pressure-lowering medicines can increase the risk of symptomatic hypotension. Strong CYP3A4 inhibitors, including ritonavir and some antifungal or antibiotic medicines, can substantially increase sildenafil exposure. Do not combine it with other PDE5 inhibitors or erectile-dysfunction treatments unless specifically directed. Seek urgent medical care for an erection lasting more than 4 hours, sudden loss or marked reduction of vision, sudden hearing loss, chest pain, fainting, or a severe allergic reaction. Sexual activity itself can place additional strain on the heart, so sildenafil should not be used when sexual activity is medically inadvisable.

Side effects and monitoring

Common sildenafil side effects include headache, flushing, indigestion, nasal congestion, dizziness, nausea, back or muscle pain, and temporary visual effects such as altered color perception or blurred vision. Less common but potentially serious problems include prolonged or painful erection, severe hypotension, sudden hearing reduction or loss, and sudden vision loss associated with NAION. Cardiovascular events have been reported, particularly in people with underlying cardiovascular risk. Obtain urgent medical attention for an erection lasting longer than 4 hours, sudden vision or hearing changes, fainting, chest pain, severe breathing difficulty, or signs of an allergic reaction. Routine laboratory monitoring is not generally required solely because sildenafil is being used for erectile dysfunction, but relevant underlying medical conditions may require clinical monitoring.
