Sildenafil selectively inhibits phosphodiesterase type 5 (PDE5), an enzyme involved in breaking down cyclic guanosine monophosphate (cGMP). During sexual stimulation, nitric oxide increases cGMP in erectile tissue. By slowing cGMP breakdown, sildenafil promotes relaxation of smooth muscle and increased blood flow into the penis. Sexual stimulation is still required for this process to produce an erection.

Important warnings

Do not use sildenafil with nitrates such as nitroglycerin or isosorbide medicines, or with recreational nitrites ("poppers"), because the combination can cause a dangerous fall in blood pressure. Stop sexual activity and seek medical care for chest pain, dizziness or nausea during sex. Side effects and monitoring

Common adverse effects of sildenafil include headache, flushing, indigestion, nasal congestion, nausea, dizziness, and visual changes such as altered color perception or blurred vision. Less common but potentially serious problems include severe allergic reactions, significant low blood pressure, sudden hearing loss, sudden vision loss associated with conditions such as NAION, cardiovascular events, and priapism. An erection lasting longer than 4 hours requires urgent medical treatment because prolonged priapism can permanently damage erectile tissue. Patients should also seek prompt medical advice for sudden hearing or vision changes. Routine laboratory monitoring is not generally required solely because sildenafil is being used, but relevant medical conditions and interacting medicines should be reviewed clinically.
