Medicine overview
Abritiga 250 mg Tablet contains abiraterone acetate, an androgen biosynthesis inhibitor used to treat certain forms of metastatic prostate cancer. Abiraterone blocks CYP17, an enzyme needed for the body to produce androgens such as testosterone. Lowering these hormones helps reduce stimulation of prostate cancer cells. Treatment is generally used with prednisone or another prescribed corticosteroid and continued androgen-deprivation therapy when appropriate. Abritiga is an oral prescription medicine manufactured by Zee Laboratories Ltd.
What it is used for
Abiraterone acetate is used with prednisone for metastatic castration-resistant prostate cancer, which has spread beyond the prostate and continues to progress despite treatment that lowers testosterone. It is also indicated for metastatic high-risk castration-sensitive prostate cancer. Patients who have not undergone bilateral orchiectomy generally continue treatment with a gonadotropin-releasing hormone (GnRH) analogue while receiving abiraterone.
How it works
Abiraterone acetate is converted in the body to abiraterone, which inhibits the CYP17 enzyme. CYP17 is involved in androgen production in the testes, adrenal glands, and prostate tumour tissue. Androgens such as testosterone can stimulate androgen-sensitive prostate cancer cells. By reducing androgen production, abiraterone helps limit this hormonal stimulation. CYP17 inhibition can also increase mineralocorticoid activity, contributing to high blood pressure, low potassium, and fluid retention; this is an important reason corticosteroid therapy accompanies treatment.
How it is taken
Take Abritiga exactly as prescribed. Abiraterone acetate tablets are taken orally as a single daily dose on an empty stomach, with no food for at least 2 hours before and 1 hour after the dose. Swallow the tablets whole with water; do not crush or chew them. Prednisone or another prescribed corticosteroid is taken as directed as part of treatment. If one daily dose of abiraterone or prednisone is missed, take the usual prescribed dose the following day; contact the healthcare provider if more than one dose is missed.
Important warnings
Abiraterone can cause high blood pressure, low blood potassium, and fluid retention, which can contribute to serious abnormal heart rhythms. Blood pressure, potassium, and signs of fluid retention require regular monitoring, with particular caution in people with cardiovascular disease. Severe and occasionally fatal liver injury can occur, so liver function must be checked before and during treatment; baseline severe hepatic impairment is a reason not to use abiraterone under the referenced prescribing information. Adrenal insufficiency can occur, particularly if corticosteroid treatment is interrupted or during infection or physiological stress. Abiraterone with prednisone/prednisolone should not be combined with radium Ra 223 because increased fractures and mortality have been observed. Strong CYP3A4 inducers can reduce abiraterone exposure, while abiraterone can interact with CYP2D6 and CYP2C8 substrates. Severe hypoglycaemia has occurred in patients with diabetes taking certain medicines, including pioglitazone or repaglinide. Seek prompt medical attention for severe weakness, faintness, irregular heartbeat, significant swelling, shortness of breath, yellow skin or eyes, dark urine, or severe nausea or vomiting.
Side effects and monitoring
Common adverse effects include fatigue, joint pain, high blood pressure, nausea, swelling or fluid retention, low potassium, hot flushes, diarrhoea, vomiting, cough, headache, and upper respiratory infections. Laboratory abnormalities can include anaemia, elevated alkaline phosphatase, high blood glucose, abnormal cholesterol or triglycerides, lymphopenia, and hypokalaemia. Serious problems include severe liver injury, adrenal insufficiency, marked hypokalaemia, fluid retention, cardiac rhythm disturbances, and severe hypoglycaemia in susceptible patients. Monitoring includes blood pressure, serum potassium and fluid-retention symptoms at least monthly, along with scheduled liver-function tests; blood glucose requires additional attention in patients with diabetes.
Pregnancy
Abiraterone is not indicated for use in females and can cause fetal harm and pregnancy loss based on its mechanism and animal data. Women who are pregnant or may become pregnant should not handle 250 mg uncoated tablets, or broken, crushed, or damaged abiraterone tablets, without protective gloves. Males with female partners of reproductive potential should use effective contraception during treatment and for 3 weeks after the final dose.
Breastfeeding
Abiraterone is not indicated for use in females. There are no data establishing whether abiraterone is present in human milk or what effects it may have on a breastfed child or milk production.
Food and drink
Abiraterone acetate tablets must be taken on an empty stomach. Do not eat for at least 2 hours before or 1 hour after taking the medicine. Food can substantially increase and make abiraterone exposure more variable, which may increase adverse effects. Swallow the tablets whole with water.
Storage
Store at controlled room temperature, 20°C to 25°C (68°F to 77°F), with permitted excursions between 15°C and 30°C (59°F to 86°F). Keep out of reach of children. Pregnant women or women who may become pregnant should use protective gloves if handling broken, crushed, or damaged abiraterone tablets.
Frequently asked questions
Why do I need to take abiraterone on an empty stomach?
Food can substantially increase the amount of abiraterone absorbed and make exposure unpredictable. Take it without food, with no food for at least 2 hours before and 1 hour after the dose.
Why is prednisone prescribed with abiraterone?
Abiraterone changes steroid-hormone production and can cause effects such as high blood pressure, low potassium, and fluid retention. The prescribed corticosteroid helps manage these hormonal effects and is an established part of abiraterone treatment.
Can abiraterone cause fatigue?
Yes. Fatigue is among the commonly reported adverse effects. Tell your healthcare team if tiredness becomes severe, persistent, or interferes substantially with normal activities.
Does abiraterone affect the liver?
Yes. Abiraterone can cause significant liver injury, including rare severe or fatal cases. Liver-function blood tests are required before treatment and regularly during therapy, and yellowing of the skin or eyes, dark urine, or severe nausea or vomiting should be reported promptly.
What should I do if I miss an abiraterone dose?
If one daily dose is missed, take the usual prescribed dose the following day rather than taking an extra dose. Contact your healthcare provider if more than one daily dose is missed.
Can I crush Abritiga tablets if I have trouble swallowing?
No. Abiraterone acetate tablets should be swallowed whole with water and should not be crushed or chewed. Contact the prescribing healthcare team if swallowing the tablets is difficult.
What needs to be monitored while I take abiraterone?
Blood pressure, potassium levels, and signs of fluid retention should be monitored regularly, and liver-function blood tests are required. People with diabetes may also need closer blood-glucose monitoring because severe low blood sugar has been reported with certain antidiabetic medicines.
